Hi all, I have a question about medicals at Knightsbridge. I've seen the medical questionnaire form IV18 they ask you to fill in before you go (https://uk.usembassy.gov/wp-content/uploads/sites/16/2017/05/USA-MEDICAL-QUESTIONNAIRE_IV.pdf), and I've seen the USCIS form I-693 that Knightsbridge fills in and the applicant has to sign (https://www.uscis.gov/sites/default/files/document/forms/i-693.pdf), and I've read the technical instructions for Civil Surgeons (https://www.cdc.gov/immigrantrefugeehealth/civil-surgeons.html). I've read on other threads (e.g. https://www.visajourney.com/forums/topic/720502-knightsbridge-visa-medicals-experience-wednesday-the-2nd-of-october and https://www.visajourney.com/forums/topic/663623-knightsbridge-medical-visit-experience) that Knightsbridge asks you to fill in a longer medical questionnaire when you're there. Does anyone have a copy of the longer questionnaire, or can you remember what questions it asks? Thanks!
