    FORM 130-A says: 1. 10.a. Family name (maiden name) of my mother. Is this the only form that requires my mother's MAIDEN name? (her name before marriage)

    when did you file your forms? I can write B1-B2 VISITOR?

    I was incorrectly admitted to the US as a B1, but I came as a tourist B2. They didn't ask the purpose of my trip at the port of entry. My visa is B1/B2. Do I need to have my i94 fixed from B1 to B2 for my adjustment of status? What should I write on my AOS forms? VISITOR? B2 VISITOR? B1/B2 VISITOR?
