  1. I'm a us citizen through Naturalization process and the following steps are needed in order to transfer citizenship to my child and apply for us passport. The confusion comes from the fact that I'm physically in USA and They want original documents. What are the documents I need to provide? For example: 1) Do I need to mail my Naturalization certificate to Morocco? Same question goes for the DS3053 ? 2) for Divorce and Annulment Decrees/Death Certificates do I need to send physical documentation through the mail? Does it have to be stamped or just a copy from court record? 3) to show presence in usa for last 5 years, do I need evidence from each year prior to birth? Year 1 2 3 4 5? 4) All previous U.S. and previous nationality passports For This one do I send all my passports to Morocco if I have a dual citizenship? I'm very confused please refere to the following text that was given to me to help find an answer: Prior to requesting an appointment you must: Download, print, and complete the CRBA Checklist (Microsoft Word, 46 KB). Please review the individual items carefully and check them off as you complete them. Complete a Consular Report of Birth Abroad, Form DS-2029. Do not sign. Please click here for an example of a correctly completed DS-2029 Gather supporting evidence (see below list of Additional Required Documents), which may be found on the CRBA Checklist. As you complete the items on the checklist, please check the corresponding box. If you would like to apply for your child’s first U.S. passport at the time of the CRBA appointment, you must also complete a passport application, Form DS-11. Do not sign. Once the above steps have been completed, please send an email to CRBACasablanca@state.gov with the subject line: “Request for CRBA appointment – [NAME OF YOUR CHILD].” Please include a scanned copy of the DS-2029 and completed CRBA Checklist with your appointment request. Appointments will not be granted without these scanned and completed documents.Note: Each child requiring a CRBA will need a separate appointment, though you may send all the information and documentation in one email. The child must be physically present at the appointment.Additional Required Documents: One (1) original Moroccan birth certificate in French from the city hall (Commune Urbaine) of the child’s place of birth. This document must be requested within one month of the child’s date of birth; Hospital records documenting the birth, prenatal records documenting doctor’s visits during the pregnancy, sonograms; Moroccan Family book, if applicable. Photocopies of the U.S. Consular Report of Birth Abroad issued by a U.S. Consulate/Embassy to your previous child(ren), if applicable.If the U.S. parent will not be present at the appointment, the following are required from him/her: Copies of all the pages of the U.S. passport and clear copies of all pages of any and all passports in his/her possession. If applying for a U.S. passport together with the CRBA, you must have a Statement of Consent, Form DS-3053, notarized by a notary public in the U.S., or by a U.S. Consular Officer at a U.S. Embassy or Consulate overseas. We advise that both parents should be present at the appointment. However, we recognize that it may not be possible in all cases. If one custodial parent is not able to attend, s/he must submit a notarized Form DS-3053 Statement of Consent. At the time the DS-3053 form is presented, a copy of the same identity document used to notarize the form must also be provided. Alternatively, the applying parent may furnish one of the following documents: Complete court order granting the applying parent sole legal custody of the child, such as a divorce decree or other custody order Complete court order specifically permitting the applying parent to apply for the child’s passport (photocopy is acceptable) Certified copy of the child’s birth certificate listing the applying parent as the only parent Certified copy of an adoption decree listing the applying parent as the only parent Certified copy of the judicial declaration of incompetence of the parent that cannot appear in person Certified copy of the death certificate of the deceased parent American Citizen Services U.S. Consulate General, Casablanca This is the checklist: Checklist for a Consular Report of Birth Abroad (CRBA) This checklist includes all documents required to apply for a CRBA as well as a U.S. passport. Please print out this checklist to bring to your appointment with your signature and date. Many of the documents require you submit an original, a photocopy and a certified translation if the document if it is not in English. The documents should be placed in the same order as the checklist with the original on top of the copy. In addition to this list, a personal appearance by the applicant (the child) is required. ** If the application package is not in this order with all the requested documentation you may be asked to leave and come back later when you are better prepared. ** One passport photo of applicant (5 x 5 cm on a white background taken less than six months ago) CRBA Application: Unsigned, completed Application for Consular Report of Birth Abroad (DS-2029) Passport application: Unsigned, completed Application for a US passport (DS-11), if applicable. Applicants must use the form filler to complete the application form. For passport applications: Parents/guardians must come in person to sign the application for a minor under 16. If one parent/guardian cannot sign, he/she must submit a signed notarized original Statement of Consent (DS3053). The document is valid for 90 days after the date of signature. Childs’ birth certificate from the local commune/city hall: Bring the child’s original birth certificate in French and one photocopy. Evidence of parent(s) U.S. citizenship: The U.S. citizen parents(S) must present his/her current and two photocopies of the biographic page. If the U.S. citizen parent was naturalized, he/she must bring the original Certificate of Naturalization and one photocopy. Passport/Identification for non-U.S. citizen parent: A non-U.S. citizen parent must bring his/her passport or other photo identification, such as a driver’s license or national ID. Please bring the original ID and two photocopies. Evidence of name change: If either the mother’s or father’s current legal name does not appear the same on all presented documents, provide official evidence of a name change. Accepted evidence includes marriage licenses, divorce decrees, and official name change documents issued by the U.S. or a foreign government. Please bring the original and a photocopy. Parents’ marriage certificate: (if applicable). Please bring the original marriage certificate (and a certified translation if the marriage certificate is in Arabic) and a photocopy. Moroccan family book (if applicable): Please bring the original family book on the day of your appointment. Divorce and Annulment Decrees/Death Certificates: (if applicable). The mother and/or father need to show termination of all prior marriages (and a certified translation if the decrees are Arabic). Please bring the original and a photocopy. All previous U.S. and previous nationality passports. Evidence of physical presence: The U.S. citizen parent must bring evidence that he/she was physically present in the United States for at five (5) years before the child’s birth. Various documents establish physical presence can include: school transcripts, medical records, employment records, income tax returns and W2s. Medical records: Documenting the birth and prenatal records documenting doctor’s visits during the pregnancy, lab work, sonograms. Non-Refundable application Fee: The fee for a Consular Report of Birth Abroad is $100; child’s passport under 16 is $135; for those 16 and over, the fee is $165. The Consulate accepts U.S. dollars, Moroccan dirhams, or U.S. credit cards. Reminder: The biggest obstacle to a quick completion of the CRBA application process is the submission of necessary documents. If you do not bring the applicable documentation included on this checklist, we may cancel your interview and require you to schedule another appointment. The above checklist is a general guideline; depending on the circumstances of your case, you may be asked to provide additional types of documentation. By U.S. regulation, the Consulate will give you 90 days to submit any additional documentation or the case will be closed of insufficient evidence to establish U.S. citizenship of the applicant. During this period, the U.S. Consulate will respond to inquiries and provide any guidance you need. Please note that all fees are non-refundable.
