6 months if no abnormal findings. 3 months if medically cleared (ie, no active TB) after sputum culture testing. If sputum testing shows active TB, the applicant will need to go through TB treatment, then repeat the visa medical after completing the treatment.

CEAC Photograph is beneficiary 2x2, white background. Beneficiary brings the same to interview. USEM screen will check the photos. If it does not pass, no biggie, they will send you to photo room and they will take photo. (My husband's photo was offwhite photo (same as uploaded yo USCIS and NVC), but USEM screening did not accept, so he had picture taken in Embassy. (I think cost was 100+ pesos, no more than P200).
My husband's interview was last July, sharing with you what was brought (we had an FB group chat for CR1/IR1 then so, we shared experiences to avoid 221G).
Entry Screening:
O USEM Appointment Letter with Barcode
O DS-260 with Barcode
Beneficiary:
O Passport
O Passport photos 2x2 – 2 copies, white background
O Birth Certificate – PSA Original of what was submitted to NVC
O Current/Valid NBI Certificate - Original of what was submitted to NVC
O Marriage Certificate - Original (PSA Original if married in Philippines)
O Cenomar/AOM (Advisory on Marriages) issued for any recorded marriages - requested less than a year.
O Termination of Marriage/Divorce Decree – Original (if any)
Petitioner:
O Copy of US Passport
O Copy of US Naturalization Certificate (if applicable) Or copy of birth certificate - in case it is needed.
O Cenomar/AOM (Advisory on Marriages) issued for any recorded marriages - PSA Original - requested less than a year. -- If petitioner is is Filipino/foreigner and lived in Philippines for more than 1 year
O Termination of Marriage/Divorce Decree – Original (if any)
O Name Change – Original (if any)
Affidavit of Support: (Yes it is mentioned on website that this is not necessary to bring, but if she can bring, the better as it is easier to show it to interviewing officer than explaining that according to website, beneficiary does not have to bring it.)
O I-864
O Last 3 years if IRS Transcripts
O Last year - W2/1099
BENEFICIARY, PETITIONER, AOS Documents must be current and uploaded in CEAC to avoid 221G. They usually review docs days prior to interview (we notice date changes on ceac tracker for last update), so if you need to upload refresh documents, upload at least a week prior day of interview.
Just in Case documents - proof of relationship - pictures, letters, etc.

Luckily any passport photo shop in the Philippines does know what a "two by two" is if it's asked for. Also ask them to email the digital copy. They will usually do that as well if asked.

In PH passport photos are a different size. Make sure that you tell the place where you are getting the photos that you need 2"x2". My wife got the wrong size. When she showed them to me I had her go back to the shop to get 2"x2".

Passport-style (2"x2") photographs of the beneficiary/applicant (YOU).
No need for photos with your father. That's only a thing for spousal visas.

1. Correct
2. No, you DO have to bring everything you have uploaded to CEAC.
3. I'll let someone from the Philippines chip in, but generally, bring all the financial and civil documents you've uploaded to CEAC, you never know what the CO asks for.

Given Name = First + Middle names
Surnames = Last names

Right. They won't review your application until you submit all the required documents.
⚠️ It looks like you processing your application on your own, right? Make sure everything is correct, no one wants to see an RFE!.
