This is exactly what I am wondering as well. I would like to have my fiance in the United States and then apply for the i-601 vaccine waiver during the adjustment of status process so that we are not separated as we deal with the I-601 waiver.

He denied the vaccines his medical in Australia and they approved his K-1 Visa and he is in now the United States. From what I understand he will need to file the vaccine waiver during the adjustment of status process.
