  5. If your wife and stepkids are pursuing immigrant visas from Thailand, enter Bangkok in that section for embassy. Do not leave item 62 blank. It is critical that you leave the previous item 61 blank.

FYI *** It would not have been flagged as it's not considered a name change to not use your middle name on your documents. Just make sure your DS-260 and ACRO police certificate lists your full name (with middle name) in the section for other names. On the DS-260 field for Name, you must enter your name as it appears on your passport. So "SMITH, JOHN" only. Then enter "SMITH, JOHN ADAM" in Other Name(s). Note that your visa (and green card and SSN card) will list whatever name is on your passport. So unless you get a new passport with your middle name listed before the interview, your visa and US IDs will not list your middle name either. This is ok in the US, as many Americans don't use their middle names on their IDs.

  9. I still recommend booking flights only after getting the visa on hand. You have 6 months (maximum) from the date of the medical anyway to travel to the US, so you will likely have time to look for good deals on flights, even after waiting until you have the visa. If you still want to book before getting the visa, check only after you get the interview schedule from the consulate.

  13. Good for you. Good luck at the consulate. For others reading this thread, do NOT use digital signature on the I-864. Hand-write the signature and date on the form, then scan and upload to CEAC.
  14. As long as your sister does not get married, she would not age out for a family-based US visa sponsored by your mother. The question is whether she would qualify under F2A (slow) or under F2B (much slower) category. Can't answer this before the I-130 (filed by your mother) is approved. That's unfortunate, if your sister's immigration was part of the plan. That 1-year delay could spell the difference in whether your sister could qualify for a family-based visa in around 5 years or in around 10 years.

  16. Definitely a problem. You must submit all pages of your tax returns or tax return transcripts. If you choose to submit your returns instead of transcripts, you must also submit all W2, 1099, and other tax schedules with the 1040. From the I-864 form instructions -- https://www.uscis.gov/sites/default/files/document/forms/i-864instr.pdf "If you provide a photocopy of your Federal individual income tax returns, you must include a copy of each and every Form W-2 and Form 1099 that relates to your returns. Do not include copies of these forms if you provide an IRS transcript of your Federal individual income tax returns rather than a photocopy unless you filed a joint income tax return with your spouse and are qualifying using only your income."
  17. If your case was expedited at the US embassy in Manila, you may need to book your visa appointment yourself via the online scheduling portal once the case status shows "Ready". Create your account at USTravelDocs and see if the scheduler is available for your IV case. If it is not enabled, contact USEM directly.

  20. It must be NBI clearance. NVC will reject local police clearance.
  21. Apostille is not required for any civil document (including NBI clearance) for US visa application in the Philippines.
  22. Just to be clear on the terminology -- IF you are the US citizen, you are the I-130 petitioner and the I-864 primary sponsor. Your husband is the beneficiary of the I-130 and the applicant for CR1/IR1 spouse visa. As for your name change, no need to update the I-130 nor inform USCIS. You should use your married name on your I-864, which must be submitted to NVC after your I-130 has been approved. Your husband should also list your married name on the Spouse info section of his DS-260 form.
