    The I-485 has a section for "Other Names You Have Used Since Birth". Your wife listed her maiden name in that section, right?

I-485A needs to be submitted with or after an I-485.     The final CSPA calculation will be done by the consulate. No special form required from the petitioner or beneficiary. All the necessary information is already on the petition, petition approval notice, and visa bulletin.

    The I-94 record will be generated upon CBP inspection on US entry. No need to make a special request for it. Just check the record online maybe a day after arrival in the US.
