The letter states what was wrong with the I-485A submission. I-485A needs to be submitted with or after an I-485. This is also clearly stated on page 1 of the I-485A form instructions -- https://www.uscis.gov/sites/default/files/document/forms/i-485supainstr.pdf
The final CSPA calculation will be done by the consulate. No special form required from the petitioner or beneficiary. All the necessary information is already on the petition, petition approval notice, and visa bulletin.

Just wait for the CEAC status to show "Issued". Whether it currently shows "Ready" or "Refused" does not really indicate if the visa will be issued soon.

The I-94 record will be generated upon CBP inspection on US entry. No need to make a special request for it. Just check the record online maybe a day after arrival in the US.
