Hello everyone, I recently received my new SSN card, and I noticed that on the card, my first name is printed on the first line and my last name is printed on the second line. On the old card, the name was printed from left to right. Is this a mistake made by SSA? Should I contact them to request a new card? Thank you!

Hello everyone, I am in the process of updating my immigration status with SSA, should have done this years ago. 😰 I will be obtaining a certified copy of my US passport from the US embassy in Shanghai and sending it along with the SS-5 form to Manila. I have a couple of questions: 1. Will a certified copy of my US passport and a completed SS-5 form suffice, or do I need to provide any additional documentation? 2. On the SS-5 form, there are some questions for which I don't have answers; for example, question number 1 - as I don't have a middle name, should I simply leave that field empty? And question number 9 – as my parents don't have SSNs, should those boxes be left empty as well? Thank you for reading and have a good day. 😊
