    oh i see. Check the status of that i-485. It seems like they denied that i-485 and put you in removal. Hence, USCIS can not issue you AP.

    you can expedite if you fall one of the categories that USCIS listed. You do not need to fill out part five which is re entry permit.

    Just ignore the stamp on your passport. Your AP expired today and that's the correct date. Do not travel unless you get the new AP documents. AP/EAD are the simplest form to fill. Just do it your self. 3 years rule will apply.

    I think OP's main concern is EAD/AP. Dan it is surely long time to wait for EAD. What category did you or your employer put on EAD application when you filled it? did you put c09? If yes then at this point i would reach out to congress person for help.
