If yes then reach out to your congress person they will be able to help you. I had same issue with my Ap/EAD receipts. My congress person was able to help me to get the receipts.
It depends where you send it. To your field office or NBC.
When you send to field office it also depends on how busy they are. I sent in NYC and i took them 8 days to make a decision.
In a nut shell there is no set time exactly how long it will take. I have seen for some people took 2 days and some for 3 months.
Just hang in there. You have done the hard work which is VAWA approved. Just wait you will get the good news soon.
it is different for everyone. Right now it is taking almost 33 months for USCIS to process VAWA. But my experience is that it is taking around 26 to 30 months.
what you can do it to ask your congress person to get you an update. They might tell you it is still within normal processing time farm. But nothing hurt to give a try.
When was your case transferred to New York? Most likely RFE was sent my local field office. But how do you know it was for medical? I would say wait until you receive RFE to see what they are requesting. involve senator as well

no you do not need any emergency reason for AP. Just for your information- if you have an emergency then you can expedite your AP.

Since you submit those documents that's perfectly fine. I agree with you that they do that deliberately so they can charge us more.
You can use fee waiver if you are going through financial hardship. The documents you need are - i-765 application, 2 passport size pics, copy of current EAD, fee if you are paying.
Is your VAWA approved? are you filling c09 or c31? If C31 yes then you have to pay.
if filling c09 based on pending i-485. Did you pay for i-485? if yes then it is free if you did not pay for i-485 then either pay the fee or use the fee waiver.
