The OP cannot meet all the requirements. Once the visa is endorsed she claims the US as her primary resident. You cannot have a primary resident in both places. Also by claiming to be a Canadian resident is one rabbit hole she does not want to go down, it's a good way to lose her US PR. Her date that she stops being an Ontario resident will also be on her exit taxes.
It is also wise to not encourage someone to use OHIP when it won't be you that has to pay OHIP back.
Who qualifies
With certain exceptions, to qualify for OHIP, you must meet all of the minimum qualifications listed below plus at least one of the additional requirements.
To meet the minimum qualifications you must:
be physically in Ontario for 153 days in any 12‑month period
be physically in Ontario for at least 153 days of the first 183 days immediately after you began living in the province
make Ontario your primary residence
