  1. Looks like I'm in the clear; thanks everybody. Will update once I'm further along in the process. @Scandi the form says page 1 of 2, however, I was told that at the port of entry (LAX) the officer only took the second page. Why that is, I'm not sure but the first page has all the information you need to verify that an examination took place from an accredited/approved physician.

  2. My wife is going to apply for AOS and I am stuck trying to figure out if I have to get robbed by a doctor or not. The sticky at the top of this forum titled "Immunizations. DS-3025 or I-693?" indicates that because she received one shot in each required series (recieved tdap, mmr, varicella) and had waiver A applied to the rest with an F waiver for Influenza. Unfortunately my wife only has page one of DS-3025 as page 2 was taken at the port of entry. Page 1 however lists the summary with the box checked for US vaccination requirements complete, the panel physicians name (typed), the date, and the signature. Will page one suffice in lieu of submitting I-693? I was told that if they require more information they will simply request it, but I'm curious to know if they would reject her application because of this and whether or not page 2 also needs to be included. Thoughts?

  3. You just enter the address exactly as you write it to send her mail. And the only consulate in Mexico that deals with the K-1 is Ciudad Juarez. It will go to Juarez no matter what consulate you list on the I-129F.

The information is much appreciated. I knew everything went to Juarez, just wanted to ensure that the information listed was correct. Also, there arent enough boxes on the I129-F to list her address. US addresses require that you list the street with number, city, state zip code, however the address in Mexico has an extra line item for the "colony". I attempted to look it up on Google Maps for clarity, but it actually got her address wrong! ;)

  4. I was filling out the I-129f and have run into a bit of an issue with regards to my fiance's address. She lives in Mexico and the boxes for her address do not allow me to properly input her address. For example, her address is along the lines of: 4 Privada Palma 22, Colonia, Cuernavaca, Morelos, and her postal code. How would I go about putting this information in to ensure proper deliver? Also, for question 36a/b, can she put the closest embassy to her or is there a specific one? Any help with this is greatly appreciated. Im anxious and don't want to make any mistakes :)
