  3. A 5 year ban prohibits entry into the US for any reason. You will need a waiver if the ban has not expired. Sounds like the CO will contact you after he verifies that a ban is (or is not) in effect.

  7. As an IR case, your PD is irrelevant. The relevant date is your DQ date. Once DQ'd, the case was placed at the back of the queue for your consulate at NVC. Once your case reaches the front of the queue, NVC will schedule the interview, notify you, then send the case to your consulate. Sometimes, it can take several months or longer.

  10. MYchart sounds like a CVS There is no national health care portal in the US. I think CVS uses Mychart. These databases are provider-network specific.

  15. If you were not required to file taxes, you don't need anything in lieu of them. Tax return/transcripts are not used to determine income unless you are self-employed. For proof of current income, you should use pay stubs, letter of employment, etc.
