    I'm confused how to answer this part of the question for my spouse as well as myself. If someone could help please?

    In the information being requested named:

    "Applicant's employment last five years (If none, so state) List present employment first."

    1. The company that employs me has moved location within the last year and has a new address, do I just use the current or do I need to enter in the old as well?

    2. Also the company that I physically work at is at one location while the cheques I get come from another main location, which do use?

    In the information being requested named:

    This form is submitted in connection with an application for:

    Would I mark the box "Other (Specify)" if I am the US citizen? If so what do I write in the Specify box next to it?

    Lastly,

    In the area for Parent's information if parents are deceased, would I just write in all the available information and in the box "City and Country of Residence" write deceased?
