I done it,3 months ago and nobody said anything about cover letter.They filed all info into the form ,packed it in a big envelope witch you dont open and gave me one copy to keep it for myself.I hope that helps
And also you're gonna need a lot of vaccination app if you dont have them in my case it was like 10 of them.:/ Still this month I have two of them left and that's it
