US Immigration from Japan

I have a few questions for the DS 5540.

For question 4A asking about insurance in 30 days: is travel insurance allowed? We were thinking about getting travel insurance until we get married and I can have my fiance added to my health insurance through work.

For part 3, my fiance and I will be living with my parents for a few months until we rent our own place. Would my parents be listed under household size?

Do I answer question 12 yes, no, or leave blank? My fiance has never lived in the US, so she has clearly not received any public benefit. The way this is worded is confusing though so I'm not sure how to answer.

Lastly, is there any supporting evidence needed, or do we just need to print and sign this form and that's it? What I've seen seems to indicate no supporting evidence is required. If I should have supporting evidence, what would I need?

I realize since this form is so new this information may not all be out there, but any advice is greatly appreciated.

The P3 states that foreign documents like police certificates and birth certificates need a "certified translation." What exactly does this mean? What organization do we need to have translate these documents? My fiancee is in Toronto.

Also, the Japanese police certificate is rendered invalid after being opened (according to the Japanese embassy), but according to the P3 this needs to be translated... not sure if I should open it and have it translated or not... Anyone have any advice for this? I emailed the Montreal Embassy but have not heard anything back.

How should I list my commission on the i-134? I have a quota, which if I hit I will make 100% of my commission. I could make more if I go over my goal, or make less if I don't hit my goal. Do I just add the 100% goal to my base salary and list that as my yearly salary?

Also, if I do a cosponsor, do we both fill out separate I-134s? is there any difference in how the sponsor and co sponsor form should look?

So, today I noticed that my date changed from September 5th to February 23rd (the date above In Process at the USCIS site). I m just wondering if this means anything? I ve been told that I can expect an approval soon. Is this date change for all case types? Or is it limited to IOE or another type?

For what it s worth, I m at Nebraska with an IOE receipt on July 31st.

We just got married last Valentines Day hosted by Cleveland Municipal Court. We were even featured on the news:

My question is:

After the event, they gave us a copy of the Certificate of Marriage from cleveland municipal court with the sealed resolution and our signed marriage license.

Since we're trying to send our papers before the 24th of Feb and we haven't obtained the certified copies of the marriage certificate yet,

Would the aforementioned marriage certificate given to us suffice the copy of marriage certificate asked in submitting the I-485??

We would really like to submit the papers before the 24~

Please help. Thanks
