RFE for name change that wasn't actually a name change

My husband (USC) has filed an I-130 to bring his Ecuadorian mom to the US. He received an RFE that we're not quite sure how to address, and would appreciate any advice. His mom has two last names, so her full name is (altered for privacy, but this is the idea): Julia Maria Gutierrez Rivera. On my husband's birth certificate, her name appears as Julia Maria Gutierrez (without the second last name). On all other documents her name is listed as her full legal name: Julia Maria Gutierrez Rivera. He got an RFE for "Proof of Name Change" and has been asked to show evidence of a name change (birth certificate, marriage cert, divorce decree, etc) to show that Julia Maria Gutierrez and Julia Maria Gutierrez Rivera are the same person. The thing is, she has never changed her name, so there isn't any proof of a name change.

Julia did go to get my husband's birth certificate corrected with her full name. The correction consists of a note attached to the birth certificate saying essentially "This registration is rectified in accordance with XYZ laws on X date that the last names of the parents are Morales Castillo (father's two last names), Gutierrez Rivera and not as recorded." The note is signed and has a seal on it. I'm wondering if this is sufficient evidence to address the RFE. We plan to draft up an explanatory letter to go along with the corrected birth certificate, but it's not exactly the evidence they asked for (i.e. it's not evidence of a name change, because her name has never changed), and also the correction isn't exactly crystal clear. That is, it doesn't outright list Julia's full name. It just says the parents' last names are: Morales Castillo, Gutierrez Rivera.

Has anyone else had a similar experience? Is there anything else we should do to clear up this situation? There aren't any documents that we're aware of that list Julia's name as "Julia Maria Gutierrez" to show that both names refer to the same person, though it is not uncommon for people in Ecuador to just utilize their first last name in social settings. Any help would be much appreciated!!

I-693 without medical exam in Orlando?

Okay, so my wife is in the process of applying for her AOS. Her K1 expires January 30, so it is crunch time (we would certainly like to avoid the $1000 fee). I have been guiding her through the steps of this process ever since I filed a petition for her over a year ago. It's gone pretty smoothly overall. She got her K1, moved here in November. We got married a week later. We're working on getting the AOS app in. But I keep running into a wall on one form: the I-693.

Everything I have read tells me that, since she had the medical exam with the panel physician in Ecuador, we don't need a medical exam. But as I call civil surgeon offices here in Orlando, it seems like no one is willing to sign off on the I-693 without also administering the blood tests and things which she has ALREADY received! She has the sealed envelope from the panel physician. But she doesn't have an unsealed copy. So do we just need to go to the civil surgeon office (or make an appointment) and hand them what we have and hope for the best? If we don't need the medical exam (particularly if we can get out for less money, since one income living is difficult in this town) I would like to avoid it. Any ideas or anyone know of a doctor we can turn to in this neck of the woods?

 I have 1 year and 3 months without approval of an I130 in Texas, on December 13 they sent an email to my husband saying they are doing a background check, how long does it take USCIS to finish their investigation? I am category CR1

My wife AOS interview is in Jan 30, 2020 and we are so ready but we just have a question, about the medical evaluation. Based on this post from 2015:

The I-693 is not required if you entry as a K1 Visa Holder and filed within the year, which is great but it does states that if you have incomplete immunization that we should follow additional steps pursing the immunization.

My wife was pregnant when she went thru the process that s why they did not administer the vaccine completely based on this. Our son is one month old, and she is breastfeeding.

We did sent the DS-3025 and did not got and RFEI on that.

So that said will it be necessary to have them done? I feel pretty confident that everything is going to be ok, we just want to make sure we are understanding the instructions from the letter
