Does anyone have the link to form I-693A or a copy of it. I can't seem to find it anywhere online including the VJ website.

If you have an e-Copy of it, please PM me.

Not readily available that I am aware of since it is not a form you prepare... The civil surgeons have it...

BTW, officially it is not called the I-693a, eventhough it seems most people call it that...

Note next to link I say "The link is on this page" If you check out that page on USCIS it clearly has a link to the supplement posted at the upper right corner of the page under related links. ;)

Some civil surgeons don't have it, they ask the applicant to get one.

Thanks a lot. The one on CDC.gov was what I was looking for.

I have not yet heard of any Civil Surgeons not having this form available to them. There is simply no need for the applicant to do anything with this form other than fill in their name at the top, ergo, there is no reason you will need this form beforehand.

Once the Civil Surgeon has filled in the form it will be returned to you in a sealed envelope which you then send onto USCIS with your application.
