Hello,

This was mentioned in another post in response to a question that I asked and I wanted to highlight it:

The Guangzhou consulate sent an old version of the i-864 in Packet 3. It is from '01. Their website has a section that lists the necessary forms for an i-864 (http://guangzhou.usconsulate.gov/visa_packet_forms.html) and this link goes to a new i-864. The difference between the old and new forms is pretty much just structural but there is one major difference-- the new form has no place for a notary to sign!

So here are my questions:

Which form should I use? the old one included in Packet 3 or the new one linked from the Guangzhou consulate's website?

Does the i-864 need to be notarized?

Also, my Dad is co-sponsoring my wife via the i-864 so I am wondering which form he should use.?? The one included in the Packet 3 materials is not even available online, as far as I can tell.
