I have a question regarding the form DS230 #14. My wife (USC) took my name after we married, question #14 ask for maiden or family name. Therefor we decided to put her maiden name. I read in another forum people using the new family name but why would they mentioned maiden family name.

Thanks for your opinion

You are correct, put her birth maiden name there, that is what is being asked for on the form, this is for name checking.

Also 21a and b your wife (USC) is not immigrating with you if traveling to the USA with you, US Citizens are not counted for question 21a and b.

OUR TIME LINE Please do a timeline it helps us all, thanks.

Is now a US Citizen immigration completed Jan 12, 2012.
