Timeline

Hello. We received an RFE for I-693 and Suppliment. We provided a sealed envelope, from the physician at the POE when we entered the USA. Obviously we can't get that sealed document back. And we don't have a copy of the I-693. We do have copies of all the test that were done to satify the I-693. Do you think that sending these documents will suffice?

Also received and RFE for birth Certificate. We sumbitted her Thailand ID card and records from gradeschool that show her birthdate and parents names and it was sufficient. Now, they want the birth certificate or a letter from the Thai Agency, on Agency letterhead, that states the Birth Certificate is not available. It would have been easier to get this doc while she was still in Thailand. Has anyone ever had to submit this document? I could really really use a sample of what it is suppose to say.

Hello,

Your sealed envelope contains the DS3025 form filled out by the overseas USCIS-approved physician after the physical exam. You could submit this, but as posted by others in the past, it is not the best way. You should take this evelope to a stateside USCIS-approved physician (lists are available at the website) and have the I-693A form completed by that person, who should give you a personal besides a sealed copy. This is the vaccination supplement that is being requested.

A K visa didn't require the vaccines but the AOS does so you will need the 693 suppliment for the vaccines. Contact a CS in your area and they will let you know what is required for the suppliment only. The TB test may be needed also.

If I am correct, (I did not accompany Maria on her K-1 based vaccination supplement medical office visit where the envelope was opened and then resealed by the stateside physician), then the vaccination supplement visit for the I-693A should accomplish both medical-related requests for hahuhobrad.

The medical was performed in September of last year. They are asking for both the I-693 and suppliment. The duplicate original provided by the physician in Thailand includes Everything. All test to include TB, X-ray, Vaccinations. But no I-693 or suppliment. Can only assume it was in the sealed envelope given to the Immigration official at the POE. I never read the post that talked about holding on to these records until later.

If you haven't already, you may want to post this is in the "Sawasdee" thread in the K-1 forum; it's for Thai people and people with Thai fiance(e)s and spouses. Lots of different people post there (not just K-1s) and you may get some help with the birth certificate issue.

The 693 is a USCIS form but the medical forms from your interview were DS-#### forms. I read in other posts this occurs sometimes and there is no way around it but to go to a CS and have it all done again.

The medical that you handed over was sufficient for AOS - unfortunately it seems they have lost yours between POE and the service centre. Join the club - contact your senator and ask them to look into it, maybe they can find it. It works for saome but It didnt work for me and i have just mailed a new one, at my own personal expense, back to CSC.

This sounds like the same ####### I've posted on similar threads in the past week. As I posted a few days ago, our medical hasn't yet made it to the correct place with the service center - in other words from the POE to the service center is taking too long, and when you file AOS they kick it back to you for the medical because they don't have your complete file - we found this out last week from our congressman's office. We're now waiting for things to catch up before proceeding to answer the RFE.
