medical paperwork

Morning everyone!!

I am gathering the paperwork needed for my man's AOS, he does NOT have the medical supplement form that he needs(I knew this would happen) so.................can he get something from the doc that performed the medical?

Gav got all his vaccination info & brought it with him but the doc did not fill out a copy for him to use here, he just transferred the info onto a form for his K-1 interview.

how can we get this info? Exactly what do we need to send in with his application?

Does he still have that same vaccination info that he took to the panel physician?

If he does, a civil surgeon in the US should be able to complete the I-693A for AOS.

I think he does(to be honest I haven't even looked at his paperwork yet). What exactly does he need to submit with the application? Is it just the I-693A? Or does that get submitted when we are closer to the interview?

My head is spinning-I really enjoyed my few months of NO paperwork

You send the I693A with the I-485 packet!

We just went and got my hubby's I-693A filled out and placed in a sealed envelope for his AOS. The place was good and quick and they only charged us $35. I'm guessing you might live in or near Philly If you are interested PM me, and I'll get the info from him.

Make sure that your SO has had a Tetanus shot, before going...they specifically asked for that shot, before they completed the form. If not, they will want to try to give it to you, or charge. Most family Drs. can give this shot, just make sure you get proof Goodluck all.
