To any members or future VJ members living in the St. Louis area, we used the following civil surgeon to get the I-693A vaccination supplement. It was quick and easy and only $50.00 (cash or check). No medical examination was required.

Forest Park Medical Clinic, Inc.

Dr. James Greenwald

1034 S. Brentwood Boulevard

Suite 1250

St. Louis, MO 63108

(314)367-6600

Note: this office is located in the University Club Tower; 12th floor. There is a parking garage and the cost is $1.00/hour for parking, or one can park across the street at the St. Luois Galleria shopping mall and walk across.

It is good to hear positive comments about a civil surgeon, maybe you could get the Captain to pin this. Thanks for sharing!

Here's my recommendation from the Austin area:

Dr. L.E. Arnold

4534 Westgate Boulevard, Suite 106

Austin, TX 78745

Tel: (512)444-3131

Fax: (512)447-4699

Not only did I receive the I-639A filled out and signed in a sealed envelope, they also made a copy of it for my records, and mailed everything back to me directly within a week. The cost was just $10.00.

NOTE: I provided a copy of the DS-3025.

Hi Omoba,

I called around to a few different civil surgeons in the area. The first price I got was $50.00, the second price I got was $50.00 and the third price I got was $110.00 - medical exam only, they would not do the supplemental I-693A.

The three I contacted are listed below. The first one is the one we used.

Forest Park Medical Clinic, Inc.

Dr. James Greenwald

1034 S. Brentwood Boulevard

Suite 1250

St. Louis, MO 63108

(314)367-6600

West Park Medical Clinic P.C.

Dr. Raymond Leung

1034 S. Brentwood Boulevard

Suite 660

Richmond Heights, MO 63117

(314)727-8099

Price: $50.00

People's Health Center

5701 Delmar Boulevard

St. Louis, MO 63112

(314)367-7848

Price: $110.00 - would do medical exam only.

Note: There are several 'People's Health Center' doctors listed on the approved civil surgeon list for the STL area. The lady on the phone at the Delmar Boulevard location listed above told me that it was the policy of the People's Health Center to do the medical exam only. Whether this applies to her specific location only, or to all the STL area locations, I cannot answer.

My advice: call them all again to get accurate service descriptions and prices. Please double check my recommendation to make sure the price, location and service offered is still the same! I just wanted to give STL folks out there a recommendation based upon our experience.

STL HCMC did you try different ones from the list in the area and what was the reply? Or were you just lucky with the first one? Any cheaper ones? Good idea to get a VJ recommended list going.........EWOK please, please!

We too received the sealed envelope, a photocopy of the I-693A, as well as a receipt with notification that if USCIS were to lose this document, we could return within one year to the medical clinic and they would produce the sealed document again free of charge.

There are not very many Civil Surgeons in Mississippi. I called the closest one that is about an hour drive away. They had never heard of just doing the vaccination supplement for the K-1. They talked with the Dr and told me if I would bring in some proof that he does not need the full exam that he would do the supplement for 50 dollars. We got there and he transferred the vaccinations to the other document and when we went to check out, he had marked on the paper to only charge 25 dollars. We were pleased with the office and the Dr was very nice and respectful, as was his staff. His name is Dr Steven Brandon, located in Starkville, Mississippi.

Hi Carla,

Glad the post helped you. I'd still advise calling around. There were some other civil surgeons in other parts of STL that I didn't contact. The $50.00 sounded reasonable to us and I didn't really have the time to phone them all and scour the market. Convenience in location was a big plus for us, and the price happened to be reasonable as well.

Thank you SO much for posting that STL_HMCM, I'll be needing that soon. You've saved me a lot of phoning around!
