DS 3025 should have been given to you when you took your medical.

It's your vaccination record.

You'll need it to adjust status when in the uS.

Yep you need your vaccination record from the medical and the I-693 form filled out and signed by a civil surgeon listed on the USCIS website for your area. If none in your area there is the Dr Arnold in texas that does them by phone/fax/mail.

Your I-129f was approved in 5 days from your NOA1 date.

Your interview took 67 days from your I-129F NOA1 date.

AOS was approved in 2 months and 8 days without interview.

you are saying that My Fiancée should have received a copy of DS-3025 after her medical exam? or it is in the brown packet she received to hand over Immigration officer?

I saw some postings on VJ where some people saying they did not receive it. Then what? do we have to take a vaccination record from my Fiancée's country and present it to the Civil Surgeon?

My fiancée asked for the second copy and he gave it to her with the original stamp.

Does she have a copy of her vaccination record from iran? if she does, Civil Surgeon may also accept it, or he may redo it again.

brown envelop that you are talking about is also contains her medical record too, but also includes other stuff.

In my case I'm willing to take the chance and not to send the Civil Surgeon report and I-693 and just ignore that form and wait to see if they REF her. and just I will send the vaccination record without I-693 form. because they already did full examination in Turkey and they have the report in their computer why do they need another examination and blood test and etc.

Please correct me if I'm incorrect.

hi there,

the best would be if a civil surgeon makes a transcript of the information from the vaccination record to the I693 - I was charged only 45 $ for it and for one vaccination that was expired. I used my vaccionation record from Bulgaria. But I saw that some people have sent only the vaccination record and they have not received RFE.

The K1 precess took us 9 long months.

09/25/09 - POE: JFK

10/05/09 - married on K1 visa

11/23/09 - filed for AOS, EAD, AP

12/21/09 - Biometrics

02/01/10 - received work permit and adv parole

02/11/10 - Card production ordered!

02/23/10 - Green card received

Something you have to remember about choosing whether to do the DS3025 or I-693:

Every required shot for AOS must be recorded on the DS-3025.

Just because you doctor in your home country filled out the form, it doesn't mean you met all the requirements. He recorded from your records and maybe shots he gave you at the examination. Some countries don't even have all the immunizations to give. You can get a K visa without having all the shots. If you know you've had every shot and every booster (for your age) needed by the time you file AOS, and it is all recorded on the DS3025, then you can use that for AOS.

If you have boosters due (in the US) by the time you file AOS or if you maybe need HPV or something after arriving in the US, then you must get everything put on one form (I-693) by a civil surgeon. He can look at what you have recorded on a DS3025, your personal records from your country, and shots you may have gotten at the health department or doctor in the US. Then he combines all of those records on to one form and seals it.

This is the latest list of what adults need for AOS. Pay attention to the age requirements. The chart starts on p. 2. There is detail about all the shots in the footnote (small print.)
