How to AOS with No Medical Record?

My fiancée will be here in a couple of month to get married and then I will file for her AOS. But she never got a copy of her medical exam (it was done, just she didn't get a copy of all the forms) and she didn't get any vaccination shots before her K1 interview. She did get some vaccination shots few years ago, when she studied in UK, but has no proof of it.

I understand we need form I-693 filled out by a civil surgeon, how can we get this form completed without any medical record? Does she need to re-do her medical exam here in the Stats, as well as have all the vaccination shots again? If this is the case when should we do this?

she needs to have her vaccination record, if not available, then the doctor could attempt to determine what vaccinatins have been done (via titer test) and then complete as needed...

Her foreign medical exam should be a part of her "A" file and need not be re-done unless it has been more than one year has passed when you submit the I-485 AOS application...
