DS-1858 Sponsors Financial Responsibility

Must I submit a DS-1858 or is that an old requirement? Its a form called Sponsor's Financial Responsibility under SSA Act.

I was never sent one and cannot find anywhere to order it from USCIS.

I found a copy online I can print and send?

Not all consulates use this form, although I think I've read of at least one that does but under a different designation (DS-2000, which is also the information form for evidence that may be presented to meet the public charge provisions of the law).
