vaccination suppliment for adjustment of status

Timeline

I dont understand the medical exam documentation. My wife had her medical exam before she came to the US on the fiancee visa as the normal step. In the adjustment of status instructions, it lists that if this was the case, then you only have to file the I-693a vaccination. What is this????? I cant find it on the USCIS website.

Please help!!!!

Theres some new rules if the date of her medical was after June 1.

There is no vaccination "supplement". There is the form I-693 which includes a vaccination section. That vaccination section has to be filled out by a "Civil Surgeon".

If your exam was done recently (less than 1 year ago), you won't need to repeat the whole exam. But with the new rules regarding vaccinations and TB testing coming into effect in June and August, you will have to get the new vaccines and TB test (if not done overseas), and then get the vaccination section filled out and part 5 signed.

my wife took the health exam between june and july 31st in krakow. According to the chart - she doesnt need to get the new vaccines?

Does this mean we dont need to file the I-693(a) for the AOS??

It means she doesn't have to get the newly added vaccines. If you read the newly added vaccines in the list below, you will find that most are probably not required for a person her age. Most likely Tdap and HPV are the only new ones added that would apply to her.

She (or rather her Civil Surgeon in the US) has to use the form I-693 dated 4/2/08 or 6/5/08. The I-693A is out of date for medicals after June 1.

Significant changes to the immigration vaccination requirements since the last revision of the “Technical Instructions to Panel Physicians for Vaccination Requirements” are―

• Rotavirus vaccine should be given orally to children 2 through 6 months of age.

• Hepatitis A vaccine should be given to children 12 through 23 months of age.

• Meningococcal conjugate vaccine, specifically tetravalent meningococcal conjugate vaccine (MCV4) should be given to persons 11 through 18 years of age. Meningococcal polysaccharide vaccine (MPSV) or other forms of meningococcal conjugate vaccine (e.g., monovalent MCV) is an acceptable alternative if MCV4 is not available.

• Human papillomavirus vaccine should be given to females 11 through 26 years of age.

• Zoster vaccine should be given to persons 60 years of age or older.

• Hepatitis B vaccine should be given from birth through 18 years of age.

• Influenza vaccine should be given annually to children 6 through 59 months of age. It continues to be required for adults 50 years of age or older.

• Acellular pertussis-containing vaccines are available for use in persons at least 10 years of age. The adolescent and adult formulation of tetanus and diphtheria toxoid and acellular pertussis (Tdap) vaccine is given as a single (booster) dose for persons 10 through 64 years of age who have completed the recommended childhood diphtheria and tetanus toxoids and the pertussis/diphtheria and tetanus toxoids and acellular pertussis (DTP/DTaP) vaccine series.
