Timeline

What do i need to be sending in for the I-693 requirement of the K-1 AOS? The Guide says that K-1 visa's only need to send in the vaccination record, but i've seen other posts that say you need to have the form filled out by a Civic surgeon? Anyone know which is accurate?

Assuming it's just the vaccination record, do i send in a copy of the record or the original?

the guide needs to be updated, the AOS requirements have changed over the summer. send in the I-693 filled out by the CS inside the sealed envelope....it couldn't hurt to send in a photocopy of your vaccination record too. looking at the amount of lost medical files...i'd be sure to make a copy or two of everything you send them incase you need it later.

It seems that there have been lots of RFEs lately due to not sending in I693. Also it seems that lots of medicals have been lost and people are being forced to get the whole medical done again.

i had my I-693 completed yesterday.

I Filled in the first part of I-693.

and went to our nearest civil surgeon and had my additional vaccination. The civil surgeon will transcribe your previos vaccination record to I-693 and seal it in an envelope.

You can give photocopy of your vaccination record to the civil surgeon and that will be sealed in the envelope too with your I-693.
