Super confused I-693 vaccination

I am really confused on what to do with I-693...

I am on K1 visa and i already had my medical exam June 2008.

some says i just need to fill up part 1 and attach the vaccination documentation worksheet... and its good to go.

while others say, i still need to go to civil surgeon and let him complete the I-693 Part 2 and get certificate.

1.) Which is which?

2.) and where can i find that designated civil surgeon? can u pls give me the website?

Actually it's your choice. There are some who didn't got any RFE by just submitting the DS form.

It won't be enough to just attach your vaccination worksheet. Your vaccinations are not complete. The Td vaccination is not available in the Philippines and you need it. Get any missing vaccinations and have a CS complete the vaccination portion of the I-693.

Prices vary widely depending on where you get the vaccinations and which CS you use.

We were able to get Marlene's additional vaccinations for free and we paid $25 to have the vaccination worksheet and record of additional vaccinations transferred to I-693 (actually I-693a vaccination supplement when we did it, but I understand the form has changed to include the vaccinations on the main form).

What they would be and what they charge are two totally different things. Shop around until you find a CS that's not a total crook and if you can get it done for under $50, that's a relatively good deal.

didn't do the medical exam because we came in on the K-1 visa. Had bloodwork done at my doctors to see what vaccinations were needed. She needed all but 1 (so she had to get 3 more) All covered under our insurance plan (under preventitive care) Took it to a civil surgeon and paid $50 for them to transcribe it.

I suggest when calling civil surgeons that you call around, the price can really vary by a lot. We paid $70 for the vaccination and the form here in Santa Ana but I have seen where people were ripped off for over $500 for the same thing

i called the nearest civil surgeon in our place and they cant give NUMBERS/fee. they just told me to go there and they will look at my vaccination record and fee will depend if i still need other vaccination.

I had mine today. $60 for paperwork, blood work and TB test... $45 for Tetanus shot, $110 Gardisil. Oh, and the 2nd office I went to gave me the Gardisil and tetanus again!!! :wow: At least they didn't charge me!!

Back to finish the paperwork on Friday when they check the TB. I think $215 is pretty good considering other places wanted $200+ no including Gardisil.

I'm in FL btw.

$230.00 total

$65 X 2, bloodwork to check for previous vaccinations

$50 for a tetanus shot

$50 for paperwork

Still the whole deal is shady and I don't trust these civil surgeon offices. They make a boat load of money; all nice and accomodating when you call to schedule the appointment, but when I called back and asked for them to explain their fees, cause they just give you a blank receipt showing you paid a total amount, not a breakdown AND I needed an extra copy of his vaccinations for our records. I had to harass them for three days to get a response!!! They accept AETNA insurance, but not for immigration!!!!!

We didn't have enough time to get the shots done using our insurance, get the results transcribed by the civil surgeon, etc. I'm just so tired with working, dealing with USCIS "stuff" and other household chores, I just decided to fork over the $230.00 and be done with it.

Sent in our AOS and EAP and AP last night.

NOA 2: 04-02-2008-->SECOND Petition approved

07-31-08- Entered the U.S.

09-17-08- Married

10-29-08: File AOS, EAD, AP

01-15-09: EAD Approved

02-26-09: Biometrics Appt.

03-07-09: EAD card received via mail

03-20-09: AOS approved

03-28-09: Greencard arrives via mail
