Can someone tell me who has used Dr. Arnold to transcribe their vaccinations records to the I-693 how long it took to get the papers back? Also, what is Dr. Arnolds contact information?

To have your vaccination information transcribed to Form I-693, the following Civil Surgeon will do it by phone and fax for a nominal fee ($10 at time of writing):

We used her - only took a couple of days (fortunately we're in Austin so we saved the couple of days by picking up the papers ourselves). Call first, I think the fee went to $15 - not sure.

We used Dr. Arnold. She is really great I would recommend her to everyone!!!

It took only a few business days after she recieved our information and we live in GA and she is in TX.

I think the fee went up to $20 but it is well worth it.
