Timeline

In Item 8 (Other Surnames Used) and Item 9 (First and Other Middle Names Used), should i write down my full name prior to getting married or should I provide the specified name for each item?

For example, say maiden name is Octavia Avalon Asimov and I married Arthur Clarke.. so my new name is Octavia Asimov Clarke. Should I just write down "Asimov" in Item 8 and "Avalon" in Item 9 or should I provide my full maiden name in both items.

oh dear, i'm really sweating this out. heheh.

hi daisychain,I think you don't have to answer item 8 and 9. When i applied for tourist visa i just wrote NONE because i don't have other maiden,first, last surnames and aliases used aside from my name in my passport.

Hi there pink just how I did it... It's not really other name because it's your maiden name now that already mentioned in your full name... if you'll write it in item 8 it's already a redundancy... mine I put none... but I think it wont be a problem if you put your last name there if you really want....so there up to you...

I read a sample guide in the internet and it says there in Item 8 and 9, I should write down my maiden name. It is redundant but that's what it asks for. I'm just not sure if I should write my full name or if I should just indicate my previous surname in Item 8 and previous maiden name in Item 9. Maybe I'll just print two Ds-156 copies, one with Item 8 and 9 blank and the other with my maiden names.

I didn't know how to answer the question, "how long do you intend to stay in the US?" so i wrote "for the rest of my life". Hahaha. My sister's employee saw it and he pointed out that I should write down "permanently" cus "for the rest of my life" sounds so melodramatic. Hehe.

I also say that ASIMOV should be in item 8 and NONE in item 9. The form is asking for the applicant's maiden name which is Asimov and not her mom's. Don't confuse a middle name with a maiden name. In the US a middle name is actually a second name while in the Philippines it's considered the same as the maiden name. The questions are not redundant and are really self explanatory.

thanks everyone for providing your insights. i read in packet 4 that incorrectly filing out the form might cause delay in the issuance of the visa. i know i'm being such a worrywart, heheh, but you never know with the US embassy. you know how meticulous they are when it comes to details and filling out forms correctly.

hmm, yah i think i'll follow onizuka's suggestion and print another one with my complete name just in case. =)
