Hi fellow vjers,

I come back again to ask some few questions about vaccination documentation worksheet. I done my physical at St. Lukes before going here and they give me Vaccination Documentation Worksheet and I still have to more shots to take here (Varicella and TD) in the US for my adjustment of Status. Now the question is:

I called and inquire every accredited doctors near our place to transfer my vaccination documentation worksheet to I-693 and do my shots of Varicella nd TD here but they said that i need to undergone another physical before they are going to sign my I-693. Do i really need to undergone physical again? What is the latest policy when it comes to transferring vaccine worksheet to I-693?

Any comments will be so much appreciated. Thanks

maybe this can help? I also don't have any idea.. I just found this in sheriffs comment...you can ask him if you want.

Adjustment of Status Tips:

Tip # 1 - Get married within 90 days of arriving into the US

Tip # 2 - Get any required shots that are not available in the beneficiary's country (for example the TD shot is not available in the Philippines but is a required to complete the vaccination report for the AOS process) then have your DS-3025 transferred to the I-693A Supplemental Vaccination Record before 90 days after arriving into the US. NOTE: If you go to your own doctor to receive a vaccination shot. Have the doctor write you a note indicating that you have received the specific vaccination shot and make sure the doctor signs and dates that note. Take the note along with your DS-3025 to the civil surgeon who will transfer the information to the I-693A (the civil surgeons have the form so you don't have to supply it). Remember, you came to the US on a K1 Visa whereas a medical exam was manditory and expires one year from the date of the examination. In other words, do NOT let the civil surgeon talk you into getting another medical exam. If you have a problem, show the civil surgeon the I-485 instructions that state that K1 Visa recipients are NOT required to take another medical exam, except for if it's been a year since the last examination.

hello there,

I then sent them to Dr. Arnold in Texas with a 20$ (that's PF and mailing back fee). Dr. Arnold transcribed my vaccination records to the I-693 form without asking another PE.

08-18-08 - NOA for EAD,AP and AOS
08-21-08 - Received Biometrics Appointment Letter
09-09-08 - Biometrics Appointment 10am Kansas City
09-18-08 - I-485 transferred to CSC (got an email from USCIS)
10-07-08 - EAD Touched
10-19-08 - My birthday / Found out I am Pregnant!
10-22-08 - Grief, Pain, Sadness
11-06-08 - EAD card ordered; AP approved (USCIS website)
01-16-09 - AOS Touched
02-23-09 - Green Card (conditional) and welcome letter received in the mail
02-24-09 - Got the job in boyce and bynum lab
