married & maiden name

Dear all

As I am now married so I filled out all forms for AOS in my married name. When I went to civil surgeon this morning, I happened to forget to fill out the I-693a form in my married name.

Will it be a problem? Or should I go back to the civil surgeon to write the form in my married name?

I am concerned, pls respond and I shall be very grateful for the response!

Hopefully you had the I-693 filled out and not the I-693a. As of May 1, 2008 you have to use the I-693.

same thing happened with me. my form I-693 was in my maiden name but everything else was in my married name. i attached a letter (explaining that the nurses from the civil surgeon's office won't let me use my married name because i didn't have any ID with my married name in it) and a copy of my marriage cert.

I honestly wouldn't worry about it. All your documentation is going to be a mixture of your old and new name. They'll be able to connect the dots.
