Local Civil Surgeon refuses to provide I693A

Timeline

Using the convenient physician locator, I was able to find a single local provider here in my community. Unfortunately, when I contacted his office, I was advised that "the doctor only provides full physicals" and they refused to provide an I-693A. I couldn't believe the response and asked for an explanation. I was told that the doctor places his signature on the form and will only do so when he has personally completed the entire physical (for the full fee, of course!). Well, Plan B now requires us to make an 90-mile round trip to the next nearest doctor...

just keep looking.... they want to take advantage of us,because they know we do anything to have papers ready.... in my case i didn't send the supplementary form i send the form that they gave my husband at the american embassy, we haven't receive a letter yet saying they need the suplementary forms... i wil way and see what happens....

You will either get a RFE or need to have it for AOS interview.

Shop around, first one I tried had not a clue, second one did it for nothing.

But I would also report the one that refused to the USCIS, not that it will do much good.

Actually because her case got transferred to California she probably won't get an RFE. Most of the cases that go there get approved without the interview process.

8/18/06---YEEEEEEEEEESSSSSSSS! Received Green Card today!(actually it should be called

the White Card, ha)....immigration free for 2 years!!

I also encountered that same problem. I found the closest Civil Surgeon, one in Ocala. They refused to sign the I-693A, and would only do the full medical. And the receptionist was just plain out rude.

So we went to Orlando instead, and the people were much friendlier. :yes:

My wife got stuck paying the full fee from the one in Ocala. The fee wasn't that bad though, he charged $125. It was the additional blood test that he ordered that was really expensive.

September 11/08: Received NOA 1

September 15/08: Rec'd Biometrics Appointment Letter

October 02/08: Biometrics Completed

Hello

Try calling around before you drive and see if any CS will transfer and sign a I-693A. Look through the rest of the forum for a Doctor O'keefe in Seattle (although he was closing in the next few days).

We ran into the same problem (why the Vancouver consulate uses a different form then then 693A and Dept of State form doesn't make any sense, for that matter why are the forms not standardized doesn't make any sense either) I just called around in Raleigh until someone said they would do it, faxed them the record we had and although I couldn't talk them out of the $30 fee for just making check marks on a different piece of paper, they did it for us, now just waiting for it in the mail.

You could also call back to where you had your original exam done and ask them to fill out a 693A to update.

I think thats a capital idea - reporting them for attempted rip offs of the newly arrived immigrant - maybe USCIS could educate them on WHO needs a full medical under the rules and who does NOT :angry: I too doubt it will happen but its just authorised robbery as it stands now - hardly a great advertisement for the whole 'welcome to America' deal.

Its not like the completion of a form is really taxing their skills as doctors is it? The non-medical staff could transcribe it and have the Doc add his sinature after checking - easy money.

Note to self: call everyone in the charlotte area and beyond if needs be to make sure I dont get a rip off artist.

i just want to share my experienced. When I had my td shot and SUPPLEMENTAL FORM TO 1-693 done the doctor explained to me the difference between SUPPLEMENTAL FORM TO I-693 AND I-693A.

SUPPLEMENTAL FORM TO 1-693= is the transfer of record from US department of state DS 2053 ( Vaccination Documantation Worksheet from St. Lukes ) to this form. (AOS applicant's Documentation Immunization)

I-693A= is a required whole MEDICAL EXAMS.
