UK Applicants - important info for medical

Please note that if you are a UK applicant you need to ask the Doctor at whichever Nuffield Hospital you go to - TO GIVE YOU THE VACCINATION SUPPLEMENT SHEET AT YOUR MEDICAL. - it is known as the I-693.

When you get to the USA, you have to send this in with your AOS application, if you don't 90% of the time you get an RFE.

It seems that the Nuffield Doctors have not been advised to give you this, even though at the bottom of this sheet it says in small print 'Give this form to the patient'. Applicants from other countries seem to get theirs at their medical!

If you take a look in the AOS Forum you will see this coming up as a problem again and again for UK applicants who get RFE's. They then have to go and find a Civil Surgeon in their area and pay more to get the vaccination information transferred onto an I-693 form.

If I had known this before I left the UK I would have made sure I had the vaccination supplement and form from the Nuffield. Now I am trying to get it by calling the Nuffield from the USA!

26-June-2006- Social Security# arrived in the mail....YAY!!

Feb 2008 lift conditions <<<reminder to self!!<<<< went to England for a visit instead, no rush right, 90 days is a long time,LOL

The Embassy sent me a copy of mine with my passport and the brown envelope and i didnt ask for it. Not sure if anyone else has got a copy from the Embassy though.

AOS, EAD Application Timeline:

05-11-2006 Applied for AOS and EAD (day 01)

05-12-2006 USCIS recieved AOS and EAD Application (day 02)

05-19-2006 USCIS cashed checks for AOS, Biometrics and EAD (day 09)

05-20-2006 Recieved NOA-1 for AOS (day 10)

05-20-2006 Recieved NOA-1 for EAD (day 10)

05-26-2006 Recieved Biometrics appointment letter (06-08-2006 at 1pm) (day 16)

Ok, somebody jump in here and correct me if I am wrong, but I believe the I-693 goes inside the sealed medical envelope. That's why it isn't given to the beneficiary.

They SHOULD however at least give you the DS-3025, which is the vaccine worksheet. The worksheet will enable a US civil surgeon to complete the I-693 Supplement A, which is what people are getting RFE'd for.

I think this occurs in part because it really is not up to the physician administering the consulate medical to bring the K-1 beneficiary 'in compliance' for AOS. They are only doing an evaluation for the consulate requirements, which are communicable diseases. Proper immunizations are required for AOS, not visa approval.

Rebecca, apparently the one inside that sealed envelope has on the bootom of it 'Give this to the applicant'! But it seems they never do, so yes the applicant needs to ask for this I-693A form at the medical. I did not know this either until recently.

The I-693 Supplement A has been added to list in VJ of things to include in your AOS packet.

Sukie, I'm just wondering who told you they are supposed to give this to you when you have your medical?

If the I693A is indeed inside the packet, then there should be no RFE.

The anecdotal evidence on VJ is that these RFE's are inconsistent. Thereby indicating that some clinics (in many countries) are not completing the paperwork properly.

Well I never got given anything when I had my medical in January?!? I did however need another MMR which I had a couple of days after (its free at my docs and it was £35 there :angry: ) and I had to post them confirmation of this....... I guess I should just call and ask them to send it me then? Anyone dealt with this through Knightsbridge?

There used to be a long thread with discussion about the supplement that was pinned in the AOS forum. It appears to now be gone....*mutters*

If your medical is done and dusted the best you can do is gather whatever information you have regarding immunizations and bring that to the states with you. You can call Nuffield and ask them to send you what they have.

Again, it is NOT the responsibility of the consulate physicians to make certain you have your immunizations for a K1 petition. This is not what they are checking for. Sufficiency of immunizations are a AOS requirement, not a K1 visa requirement.

Below is part of a PM I received from another VJ'er who got a copy of their medical sent to them from the UK:

When I recieved the the copy it was in the brown envelope saying do not open, we opened it anyway and checked to see that everything was there, the vaccination suppliment is in there and at the bottom in small print it says give a copy to the patient (which they do not do!!!),

Please note this quote from the instructions:

Note that the vaccination supplement accompanying Form I-693 is not included on this internet posting. The Centers for Disease Control and Prevention (CDC) have sent the vaccination supplement directly to the civil surgeons for them to photocopy, fill out on behalf of each applicant, and attach to Form I-693 for submission to USCIS.

I note that Wes' vaccine worksheet (DS-3025) has the quote on the bottom 'Give copy to applicant'.

The I-693 has no such language on it, nor does our copy of the I-693A (completed by a civil surgeon in the US).

On the old forums, there was a LOOOOOOOOOOONG thread started by AgentSmith about this RFE problem when it first started to crop up last fall. There was a LOT of confusion about what is the worksheet, what is the supplement, blah blah blah. There was also a lot of really good dialogue in that thread, with many VJ members helping others get this sorted.
