Before my friend come to the US (K-1) they gave him the immunization record but not the Form I-693. According to this link http://www.visajourney.com/forums/index.ph...mp;page=k1k3aos

I-693, Medical Examination of Aliens Seeking Adjustment of Status. (If you already have this from your overseas Visa Interview submit a copy that medical check form; No fee is required in this case. If you are a K-1 fiancé(e) or K-2 dependent who had a medical examination within the past year as required for the nonimmigrant fiancé(e) visa, you only need to submit a vaccination supplement (I-693A), not the entire medical report. You may include the vaccination supplement with your adjustment of status application.)

I was looking for I-693A but couldn't find anywhere

Civil surgeon will have it.

Take the I-485 instructions with you when you do visit the doctor, many like to do a full medical, and showing them the instructions tends to convince them to just do the supplement at a substantial lower fee than a full medical.

You dont have to break your head about it. Set appointment to civil surgeon and he has to know this. He will give you sealed envelope with this form for sending in with your AOS, and copy for you too.

07/29/2006 – I-129 sent to Vermont

08/04/2006 - NOA1

08/28/2006 - NOA2 - approved

09/01/2006 - NVC - approved

09/07/2006 - Warsaw embassy sent packet 3 (damn post services, never received any)

09/18/2006 - packet 3 sent (Nothing fails)

09/27 - received packet 4

10/10 - medical exam

10/19 - INTERVIEW!

10/20 - received visa

11/7 - arrived in USA, POE YFK

1/19 - Married

02/23/2007 - Civil Surgeon (checked just vaccines for $ 25)

05/04/2007 - AOS package sent to Chicago

05/11/2007 - NOA1

05/15/2007 - NOA2 - ASC appointment letter about biometrics

05/24/2007 - RFE about tax forms w-2 and 1099!!!

06/05/2007 - Biometrics

21/06/2007 - NOA3 - Transfered to California

10/07/2007 - AOS approved, card production ordered!!!

19/07/2007 - Half year marriage anniversary - GC arrives!!!

07/08/2009 - Package sent (My cover letter 40 peaces of evidence)

07/14/2009 - check was cashed

07/10/2009 - NOA 1 received, GK extended for a year

07/17/2009 - received biometrics letter with my case number

08/06/2009 - scheduled biometrics appointment

11/16/2009 - approval

12/01/2009 - touched - card production ordered

2/26/2010 - got ten year card

He didn't get the TB skin test yet so it would cost around $200 (full medical report)
That is done for the consulate interview, TB is grounds for inadvisability, so has to be done in the foreign country.

The only things that the local Civil Surgeon in the USA will be concerned with are:

  • mumps, measles, rubella (MMR)
  • tetanus and diphtheria toxoids
  • varicella

I've called all the civil surgeon offices at my local area and they only do Form I-693. The other office say if I just want to do the vaccination supplement which it's not listed on the USCIS Civl Surgeon

P.S. on his immunization record, it shown the date given for Td, Tetanus, Diphtheria, and MMR

You can't find it because there is no form with that designation. The term is a VJ creation. Look for the I-693 Vaccination Supplement - you'll find it on the CDC web site.

What area and state are you in? You might get some CS name/cheaper fee quotation or what not by referral (based on other people's experience) if you indicate more in your profile.

Just visit the civil surgeon and show them the instructions from form I-485 stating that only the vaccination supplement is needed for K-1 holders. The doctor in my area stopped insisting on full medical when I showed hum the directions. Fee $35

I called around getting prices for how much the civil surgeons in my area would charge to transfer my vaccination worksheet DS-3025 over to the I-693 vaccination supplement and couldnt believe my ears when a lil voice on the other end of the phone said........*NO CHARGE* :o i was shocked to say the least, so as every VJ'r will tell you, its better to call around a few CS in your area . I'm glad i did, so tomorrow i have my appointment with the CS in Columbus, Nebraska then soon as i'm home my AOS application will be complete and will be in the mail and on its way by tomorrow afternoon :D

Summertime, USCIS doesn't generally give out the I-693 A (although sometime back I managed to find it and download it from their site). The Civil Surgeon has copies of this form at his office, that he is supposed to use. So call the other office (that offers vacination supplement only, hopefully for a low fee) and see what they can do for you.

05/12/2009 -- Overnighted application by USPS express mail (VSC).

05/14/2009 -- Green Card expired.

05/23/2009 --- Check cleared bank.

05/26/2009 -- Received NOA (NOA date May 15, 2009, guess they aren't deporting me).

05/29/2009- Biometrics Notice date

06/01/2009- Received Biometrics Letter

06/18/2009 - Biometrics

09/23/2009 - date of decision to approve (letter received), just waiting for card. No online updates whatsoever.
