I-693 Question for AOS of a K1 Visa holder

I'm definately getting the hang of this AOS stuff. Thanks to you guys 75% of the paperwork is correctly filled out and ready to be submitted!

I'm confused about the I-693... I've read up about it on here and USCIS... I didn't get a DS3025 with my medical. All papers were given to the POE guy. I have a print out (a screen dump) of my medical records from my doctors which state every single vaccination that I have been given (I only don't have chickenpox of which I had when I was a kid).

Will this suffice or do we have to fork out yet even more money for another medical? If so, why don't they do this during the medical for the K1 visa... grrrrrr!!! This costs soooo much!!!

If I am going to have to get the I-693 done, can anyone reccomend a good reasonable civil surgeon in the Hudson, WI area?

You'll be able to take the vaccination records that you have to the civil surgeon, who will then fill out the supplemental. You won't need a new medical.

Prices can vary for the supplemental, so shop around.

Hi Nicola,

Provided your K1 medical was completed less than a year ago, you do not need to have another medical examination. You will, however, need to get a Vaccination Supplement form (I-693A) completed by a civil surgeon. Take your vaccination history along with you, including your copy of the worksheet DS-3025 (if you had chicken pox as a child, you will need to have the results of a blood test to show you are immune). The civil surgeon will copy this information on to the supplemental form and put it in a sealed envelope for you. Ask them to make a copy for your records. I was able to get this done for free, but others have had to pay (approx $50) for the service.

You can look HERE for a civil surgeo in your area. B prepared to ring around to get a good quote, and make sure to tell them it's only the Vaccination Supplemental form you need, not a full medical
