You are not alone. Last Thursday the Doctor made Cory had the HIV test even though is no longer required since January 4 this year. We went last week and still she had to take it. Now we have to go again tomorrow so he can check the results and finish with our papers. That is life.

March 25, 2010 - Advance Parole approved..received on April 1 in the mail

March 29, 2010 - I-485 notice date, received on April 4, 2010

May 3, 2010 - EAD approved

May 5, 2010 - Date interview....approved

To have your vaccination information transcribed to Form I-693, the following Civil Surgeon will do it by phone and fax for a nominal fee ($10 at time of writing):

I just called Dr. Arnold's office and the person who answered the phone said "at this time we are no longer providing this service" (assisting people with AOS applications and DS-3025). So it sounds like DR. Arnold isn't a good contact anymore.

I just called Dr. Arnold's office and the person who answered the phone said "at this time we are no longer providing this service" (assisting people with AOS applications and DS-3025). So it sounds like DR. Arnold isn't a good contact anymore.

There was a big post about this - basically, it is believed he violated the CS rules by doing this:

To have your vaccination information transcribed to Form I-693, the following Civil Surgeon will do it by phone and fax for a nominal fee ($10 at time of writing):

Without actually seeing the people (which is in the instructions for the form).

By it's all conjecture - however, he is no longer doing this.

My Advice is usually based on "Worst Case Scenario" and what is written in the rules/laws/instructions. That is the way I roll... -Protect your Status - file before your I-94 expires.

Hi everyone,

I just had my I-693 filled out and done today. I'm living in NC and I went to this doctor in Garner, NC. The doctor's name is Dr. Gonzalo Fernandez in Occupational Health Partners. He's situated in Garner and Raleigh as well.

I had to pay USD185 for the following breakdown:

Paperwork - $15

Immunization Administration - $15

Tdap shot - $45

Varicella Booster shot - $110 (I could've done the bloodwork, which costs $70 but if the bloodwork comes out that I do not have full immunity against Varicella even though I've had chicken pox before, I would need the booster shot.... so I decided to play safe and pay $40 more now than having to pay much more later)

They're really friendly and were upfront about the payment, which is really good. I recommend this to whoever is living around here.

Hi,

I am a K-1 visa holder, about to adjust my status, and have only happened across this part in the form, I have the DS-3025 (the vaccination document worksheet) which was given to me when i had my medical in London.. all the vaccinations have been checked under the completed series, or not age appropriate... except this Varicella one. Which say VH beside it, and then at the bottom in the results section of the form they have checked the box vaccine history incomplete. They have also checked - Applicant may be eligible for blanket waivers because vaccinations not medically appropriate.

After reading this post it seems i need to get this transcribed onto form I-693 by a civil surgeon here in the US, is this correct? when i call the civil surgeons is this all i ask for?? Do i need to have this Varicella vaccine? Surely I don't need to have another full medical??

Sorry I thought i was all sorted and ready to send my forms off, can i send my AOS as it is with the DS-3025?? And if i get a RFE for further medical will it then hold up my EAD authorization? or is just better to have it all correct straight up?

Regarding the Varicella, mine was written as VH as well in DS-3025. However, the civil surgeon here in NC told me that I have to prove that I have immunity against Varicella (which is chicken pox) according to the latest USCIS-CDC requirements. So it's either take a blood test to check the full immunity or take the vaccine. I can't entirely prove that this is needed but I'm just going by the civil surgeon's advice, so I took the vaccination. You can see my post regarding it above.... which I wrote

I hope that helps a little... sometimes people do send in DS-3025 for AOS and get away with it but a lot of them are now getting RFEs as I know... so I would suggest you get I-693 sorted out to avoid any discrepancies!

The CS you dealt with lied to you or has old information.

http://www.cdc.gov/immigrantrefugeehealth/...vaccination.pdf is the technical instructions for the civil surgeon (the latest one dated Dec 14, 2009 with the latest changes (e.g. no HPV, no HIV blood test, no Zoster)). Page 11 speaks about varicella and that an oral or written history is required. I have my baby book where mum wrote it in when I got it, I also have a chicken pox scar and there's a photo somewhere. I'm sure I could also get a written thing from primary school when I took time off from being sick. That's all the proof they need.

Page 12 also mentions it being written as VH.

How would we know if my husband was supposed to have any more vaccinations? I am supposing "NO" b/c nothing was mentioned to us at the K3 interview or POE. My husband did however have another varicella in order to attend UConn. (Apparently CT requires two of these guys.)

At any rate, if he does not need any vaccinations, do we need to send any part of the I-693 at all? I'm just getting started on all this and would appreciate any guidance/advice.

Also, our 2nd anniversary is 5/8 (medical 5/4). How early do you think we can submit and get the 10yr GC? We really don't want to go through Removal of Conditions down the road. Similarily, if we have international travel plans in early July, is it safe to assume we should file the I-131 along with AOS? We are going to Europe and I really want them to let my husband back in the USA!

From everything I've heard Dr. Arnold no longer is a registered civil surgeon so no.

Ugh, the Varicella nightmare. My advice is, shop around for a good civil surgeon who will give you an inexpensive blood titre to test for the antibody. Our civil surgeon saved us big bucks (and extra jabs into my poor hubby) by insisting on it when we visited him for the I-693. He said, "why pay extra for a shot if you've already got it in your system?"

That sounds as if they are charging you for the whole medical instead of just transcribing your vaccines. Theres another thread in this forum with a list of good CS's, take a look through that one. You don't need a whole new medical if you just came in on a K1 and your medical was completed less than a year ago.

If you're still in the K1 process...then the non-USC gets the medical done at one of the assigned doctors in their country, in some cases assigned by the consulate they will be interviewing at.

It is going to cost a lot for me to get I 693 done since I'm on F1 visa and I don't have any prior immunization records (pretty sure I have had all the shots). How long does it usually take to get the results? I heard from a friend, it took him 3 weeks to get it done (about $500 bucks). I have it even worse since I go to school in a tiny town, there is only one CS nearest to me.
