We solved our problem with my husbands missing DS-3025 form. We phoned Knightsbridge Drs office in London and they faxed it to us with in half an hour. They even wrote well wishes for our future on cover letter ((Thanks Lucy at Knightsbridge!!!! And Bless you.))

FYI- I don't know about other DRs but Knightbridge keeps the records for a year. Easy to just call over and collect it but you must know date and time of your medical and have a fax. No emails.

Well, this part of the process is certainly a pain! Here is my story (still developing)...

A few weeks back, I went to a US CS and got a HPV booster for my wife and had to argue with them about what was/wasn't required for us. So they gave us the i-693, *unsealed*, with the Vaccination page filled out with *no* checkmarks in the "Not Medically Appropriate" column for the vaccines that had an insufficient time interval (basically HPV). What was also alarming is that she checked the results box "Applicant does NOT meet immunization requiremnts".

So last week having gotten through all the forms and instructions, I contacted them to see about getting this fixed, and went in today to do so (the CS was on vacation until today). Went over the issue in detail with the nurse, who talked to the CS who said she would not Tick the box for "eligible for blanket waivers as indicated above" claiming that she was following the instructions given by the USCIS for CS. So I told her to write a note and sign that all was needed to meet the requirements was the final HPV booster and the nurse relayed this back to the CS. Well, the CS came out from the back in a bit of a huff and refused to let me point out to her the details of the form and instructions (and to look at the DS 2053 which clearly showed the way the box *should* be checked), got quite rude, pointed at a notebook saying it instructed her to check the "does not meet" box, and refused to change anything or put an appropriate note (she did "concede" and write a note that at my insistence I forced her to write a note saying another HPV booster was needed).

So, now I am almost back at square one. I have to:

a) fight her more to try to get it corrected (I don't trust her to do this)

B) not submit an updated i-693 and see what happens

c) find a CS here that will transcribe for a reasonable fee (was offered a "deal" for $65 when i called and asked another CS about this).

This part of the K1 saga is stupid. I want to find out where I can complain about this Dr.

FYI: Dr. Michael Cobble, of Sandy, Salt Lake City, transcribed my I-693 for only $25, beating Dr. Arnold :) They did, however, send me to SLC Public Health Clinic to get a Hep B vaccine ($45, but he didn't benefit from that personally) that the CS in Vancouver had checked off as "Age Inappropriate".

my husband called Dr. Cobble to ask to transcribed my I-693 and get some of my second shot but he wants me to redo my medical again!!! i just had my medical this march of 2009 he said it is now required! i called the USCIS and asked about it, but the operator said i only need my vaccination supplement since my medical is less than a year... now my husband says we still go to see this doctor and have him call the USCIS...

I dunno. Medicals are supposed to be good for a year, unless you have a Class A condition, in which case they're only good for 6 months. Assuming you don't have a Class A, they may have misread/misunderstood the rule and think you need a new medical because it's been more than 6 months. My medical was at the end of April (same day as my K-1 interview, see timeline) and I had them transcribe it in early August, so my medical was not yet 6 months old at that point.

I entered the U.S. on a K-1 Visa and now am applying for the green card. I just got my medical 3 months ago and just need my vaccinations transcribed by the civil surgeon. I called Dr.Arthur in Texas and they said that they don't do that anymore...starting on Dec 1st of 2009. Do you know of any other civil surgeons that could do that for a cheap price? I live in Hawaii and they are all asking me to come in a make an appointment even, to fill out paperwork...and it would cost between $250 and $400!! Are there any civil surgeons that can just fill out my paperwork for me????

well we went to Dr. Cobble last week and we told to the assistant that we only need my follow up shot according to USCIS and he asked me to go to Public Health Clinic for my shots and come back to them, we were not able to come back to them that day as the car got broken so we come back today to transcribed my vaccination supplement to I-693 and so happy that they did not ask me to redo my physical exam and they only charged $25 but we got a discounted $5 coz the assistant dont have any change for $40... :) good thing i read this post of yours!!! hehe

I arrived in NY mid sept on a K1 visa, married early nov,.......All well in that department.

Now it's the dreaded 2nd round of paperwork.

1. Anybody know of a human civil surgeon in NY.

2. My vaccination doc worksheet (from london) has 3 "K1" entries in the MMR lines. What does this mean?

3. My K1 visa expires in a few days. How important is it to get all the AOS forms sent in before then.As i cant imagine getting to the bottom of the "uncivil surgeon" issue in a hurry.

Would the post above yours apply to you? You have your 3025 filled out, it sounds like. I skipped the 693. I may get an RFE, but quite a few people don't if they've had their K1 medical completed with all of the AOS-required vaccinations done.

Rin and John, I would like to know, my husband entered US as K1 and he did the medical exam (DS 3025) and gave the doc when he entered US. We are going to have interview next month and asking about I-693, we didnt submit I-693 as part of our I-485 package. My question, during your interview, did the officer ask for I-693? Im kinda worry whether we should do I-693 or not, cuz we didnt have copy of DS 3025 cuz the medical office in his home country didnt give any copy except the vaccination record... your reply would be helpful thanks a lot

Please let me know what to do? to redo the medical all over again is to expensive :(

We received the AOS interview letter. It has a checklist of what to bring listing the I-693 first stating ( if not already submitted). However, I sent the DS-3025 & 3026 with my AOS package and I never got an RFE for the I-693. Do I need to get on from a civil surgeon?

Some countries allow K non-immigrant visa applicants to defer their vaccinations until after they arrive in the U.S. If the beneficiary's vaccinations were not completed as part of the visa process, it is necessary to include form I-693- part 1, part 2-vaccination section, and part 5 in the application.

If the beneficiary's vaccinations were COMPLETED as part of the visa process, IT IS NOT NECESSARY TO INCLUDE FORM I-693 IN THE AOS SUBMISSION. All your medical information is enclosed in the brown envelope which was handed in at the POE. The envelope is then forwarded on to USCIS where it will be consolidated with the I-485 submission, once received. If you received a copy of the Vaccination Documentation Worksheet (Form DS-3025) from your Panel Physician, you may include a copy of that form in your AOS submission in place of Form I-693.

I think it depends on the type of "entry" you had... K1 or whatever. My husband came to the USA on a K1 and now we just have to have the vaccination transcribed, b/c his medical was done on March 2 in Toronto by Dr. Seiden. Hope that info helps. :)

The medical info should be in your file that the USCIS has (once the embassy is done with the visa process, the info that is in the brown envelope surrendered at POE goes to USCIS and contains all of your paperwork, including the medical info). Since you are at the point of having an interview scheduled and did not receive an RFE, then I would suggest just going through with the interview and assume USCIS has everything they need. If they don't have the medical info, they will tell you at the interview and give you a period of time to get it done and submit the I-693.

If you decide to go the I-693 route, it has to be completed by a civil surgeon from USCIS' list on their website. A family doctor (unless also a civil surgeon) will not be accepted.

If you decide to go the I-693 route, it has to be completed by a civil surgeon from USCIS' list on their website. A family doctor (unless also a civil surgeon) will not be accepted.
