The new rules are on the new I-693 form instructions. I posted in that topic (at the end) with a link to the new info. Now one DOES need to send an I-693 completed, just part 1 and part 2, #5.

The form I-693A supplement is out of date that I found linked here somewhere. The new I-693 form has several new required vaccines.

6/13/08 Applied for SSN for David ----- TRIP 1; 6/23/08 Went back to SSA 'cuz the person didn't pc all documents (rec'd letter from them) ----- TRIP 2

7/2/08 Received David's health insurance cards! Yay!

7/3/08 Went back to SSA; they said come back 7/9 if he still hasn't received it ----- TRIP 3

7/8/08 Got list of vaccines he still needed from CS by fax/phone; tested for immunity to MMR (only vaccine still needed) at our family dr

7/9 & 7/10/08 Went back to SSA, card still not received - Denied opportunity to apply for replacement ----- TRIP 4; Went back to SSA again to apply for a replacement card - Talked to a supervisor this time, POE people entered his last name incorrectly ----- TRIP 5; went to get David's ID thru the State, denied - instructions wrong on website; measles test inconclusive, another test needed; requested new lease

7/14/08 SSA called me in regards to problems at local SSA office

7/15/08 INFOPASS appt to fix POE name input error and then went to airport to fix POE name error

7/17/08 Received SSN card in mail and went to get state ID

7/21/08 State ID received in the mail and tested again for measles immunity

7/29/08 MMR shot #2 given and dropped off papers at Civil Surgeon (CS)

7/31/08 Picked up I-693, completed by local CS, (I paid $25, don't be fooled)

8/3 - 8/6/08 Worked on forms I-485, G-325A, I-864, I-765 and I-131; sent to Chicago - YESSS!

8/11/08 NOA1 for AOS, EAD and AP; 9/2/08 Transfered to CSC; 9/4/08 Biometrics

10/24/08 Rec'd AP; 10/27/08 Rec'd EAD; 11/24/08 Rec'd GC

The following link was INVALUABLE to me to figure out what the heck was going on with the I-693. It looks like we DO indeed have to submit the I-693, part 1 (Information about you) and part 2, #5 (vaccination section) completed for K-1 visa holders. Please read Section 3, Frequently Asked Questions, #2 for clarification.

These forms were revised on 06/05/08. New info.

The new required vaccinations include Rotavirus, Hepatitis A, Meningococcal, HPV, and Zoster, on top of those previously required.

I have posted seeking advice since my fiancee has 1 HPV shot left to take, which is now required. She cannot take it until August, though she arrives this Sunday. I am still trying to judge if all vaccinations must be complete or only started by the time we file for AOS (I have heard both sides). If the former case is true, then I guess we'll have to wait until August. If the latter is true, then it seems to me we can send in eithre the DS-3025 by itself or have it copied to the I-693 (Parts 1, 2 - vaccination supplement only, and 5).

Anyone else have any thoughts? I asked my fiancee to ask her doctor but am unsure if the doctor would even be aware of the changes.

If your vaccinations are not complete as of AOS filing you will either be RFE'd for aditional docuemnts or interviewed where you will be able to provide.

Good question! Unfortunately, I don't know... Luckily David is a guy and we don't have to deal with it. Have you tried calling one of the Civil Surgeons in your area to get his/her suggestion?

I think that could go there instead:

When you apply to adjust status in the States, assuming you as a K-1 holder apply within one year of the original medical, you need I-693.The instructions to form I-693 say: “What if I am a K nonimmigrant visa holder and had a medical exam overseas?

If you were admitted as a:

A.K-1 fiancé(e), or K-2 child of a K-1 fiancé(e), or as a

B.K-3 spouse of a U.S. citizen or a K-4 child of a K-3 spouse of a U.S citizen

C.You received a medical examination prior to admission, then

a.You are not required to have another medical examination as long as your form I-485 Application to Register Permanent Residence or Adjust Status,is filed within one year of your overseas medical examination

b.You will, however, be required to complete Part 1,information about you,and submit the vaccination secton of Part 2 with your adjustment of status application. A designated civil surgeon must complete the vaccination section and Part 5 the Civil Surgeon’s Certification

Here's where it gets tricky! Some people have submitted a DS-3025 (obtained overseas at your medical documenting your vaccinations) with their AOS package and have NOT received RFE's. Some HAVE. If you want to be absolutely safe in terms of your medical documentation as a K-1 adjusting status, you should take your DS-3025 to a USCIS-certified civil surgeon and ask them to transfer the information from your DS-3025 to an I-693. They should do this for a nominal charge. If they try to tell you you need a full medical examination, DO NOT LISTEN. You ONLY need the vaccination part. The following memo from the USCIS documents that this completed form will remain valid beyond one year when submitted with most Adjustment of Status applications.

The civil surgeon should give you an I-693 documenting the fact that your vaccinations are complete (either they were completed when you had your medical or you had them completed stateside). You should submit this with your AOS package.

That's for tip #1. It could also work for tip 2 (tip#2 also needs to be updated)- if we add the link to the instructions to I-693 form because there is all the information in the FAQ section that applies not only to K-1 visas but also a couple different ones that have already had their mdical exams overseas. With this topic http://www.visajourney.com/forums/index.php?showtopic=101202 the only changes I would make is to put "form I-693- part 1, part 2-vaccination section, and part 5" instead of "I-693a vaccination supplement" (since it doesn't exist any longer and part 1, 2 and 5 of the new form make up for the previous I-693a).

The topic will work with those little changes.

Looks good although... (forgive me but I am a crazy perfectionist). In this topic http://www.visajourney.com/forums/index.php?showtopic=101202 you have corrected only this part:

Some countries allow K non-immigrant visa applicants to defer their vaccinations until after they arrive in the U.S. If the beneficiary's vaccinations were not completed as part of the visa process, it is necessary to include form I-693- part 1, part 2-vaccination section, and part 5 in the application.

The rest of the post still has these: I-693A or "vaccination supplement" (only in about 3 more sentences) .But I thought...to make sense those need an update, we need to make the "A' and the "vaccination supplement" dissapear because they both now mean- "PART 1, PART 2- vaccination section, and PART 5"

OH! AND THE MOST IMPORTANT! THE TOPIC is named: "FAQs:Forms I-693a and DS-3025" (the "a" needs to go!)

We don't want to confse any more people!
