Hi Folks,

Down to last thing before we send in our AOS packet, the I-693A. We managed to find the form, but at the embassy, they took everything from the pre-interview medical. All we have left is her non USCIS shot record which the pre-interview Docs didnt annotate.

So what do we put on the I-693A?

You will need to find a Civil Surgeon authorized by USCIS to do the I-693A for you. They're the only one who can do it. Here's where you can find a CS in your area... https://egov.uscis.gov/crisgwi/go?action=of...office_type=CIV

Just bring the shots record so that the CS will know which vaccination needs to be administered. The CS will then give you the I-693A in a sealed envelope that you want to include in your AOS packet.

You take the vaccination record to a US Civil Surgeon, and they transfer the record to the I-693A not you.

The civil surgeon transfered my wifes shot record from her vaccination book to a supplement for $35 after I insisted that all we needed was the I-693A supplement per I-485 directions. (I showed him page 3 and pointed out paragraph 8)

8. Fiancé(e)s.

If you are a K-1 fiancé(e) or K-2 dependent who had a medical examination within the past year as required for the nonimmigrant fiancé(e) visa, you only need to submit a vaccination supplement, not the entire medical report.You may include the vaccination supplement with your adjustment of status application.

Do a search on this website for DR L. E. Arnold of Austin, TX or I693A on this forum. She will work up your I693A form for you from that shot record you have from medical exam taken in her home country. Will do it all by mail and only cost you $10. You will find all these other civil surgeons will charge you an arm and a leg and try to make you take the whole physical all over again. Good luck.
