K-1 Vaccination Supplement?

I am a K-1 Visa holder and just got married. I am from Vancouver, BC. I am now filing for my AOS, EAD and AP. However while looking at the "Section 6.0 .. After Marriage - Adjustment of Status" I got a little confused regarding my vaccinations. It says:

"The I-485 instructions mention that no additional medical is required if filing for AOS within a year of the medical required to obtain the K1/K2. You should simply send in a copy of the I693A (sheet that lists your vaccinations) that your doctor should have provided overseas when you had your medical."

I had my medical exam done and Vancouver and I supplied my vaccination records. I was given a worksheet from the doctor's office that was put in an envelope by the Vancouver Consulate. It states my vaccination records. However it is not a USCIS form I693A like the Visa Journey guide suggests I had received. Also there is no form listed as I693A on the USCIS.gov website. Does this form exist? Do I need to have it?

many thanks

This is a very common question, search "I-693A" and the answer will be revield.

Vaccination record is not required for the K-Visa, it is required for the green-card. The I-485 directions says you "MAY" include the supplement with the petition, however if USCIS decides to approve without interview you probably will get an RFE for the supplement, if not your interview letter will request that you bring it to the interview.

You take foreign vaccination record to a US Civil Surgeon and get it transfered to a supplement, fee $10 - $40, insist on just the supplement, if you have to show the doctor the I-485 directions.

You may have gotten a copy of the DS-3025 from your medical. If you have that sheet, you can simply have a civil surgeon transfer the information to the form you need for AOS. Otherwise you will need to take what information you do have to the civil surgeon. They will figure out what shots you need if any, and provide you with a completed I-693A.

Some have had success simply including only the DS-3025 in lieu of the Vaccination supplement... YMMV

I have simply inlucded only the DS-3025 and got approved with it.

Feb 2005 - APPROVED for K1 Visa

June - 2005 - Moved to the USA =)

Apr - 2006 - APPROVED AOS Without Interview!!

Feb - 2008 - REMOVAL of conditions!!!!!

02/20/2008 - Package was sent to TSC.

02/25/2008 - Package confirmed received at TSC.

Better safe than sorry, find a civil surgeon and get the supplemental done.

To quote from the I-485 Instructions:

8. Fiancé(e)s.

If you are a K-1 fiancé(e) or K-2 dependent who had a medical examination within the past year as required for the non immigrant fiancé(e) visa, you only need to submit a vaccination supplement, not the entire medical report. You may include the vaccination supplement with your adjustment of status application.

So things to point out here:

1) You "may" include the vaccination supplement... -- May does not indicate a requirement

2) There is no mention anywhere in any of the form instructions that a civil surgeon is required to do anything.

3) There is also no mention of Form I-693A on any of the instructions for any of the forms.

Not arguing with folks that have gone through this already but USCIS is leaving a lot of room for interpretation here. The embassy took all the papers we had including the vaccination forms from the K-1/K-2 medical exam, so theoretically those are already in our packet at USCIS waiting for our AOS application.

Got to love federal ambiguity ;)

Do you have a copy of the worksheet from your doctor? If so, you can include that in your AOS package, and hope you won't get an RFE. I had one of those little Canadian booklets that has the vaccinations on it, which I showed to the panel physician in Canada, and again in the US when getting my supplement done. I had it done again so I wouldn't get an RFE.

You could also try sending an email to the civil surgeon in Canada, requesting they send you a copy of the vaccination sign off sheet that he filled in at the exam, and you could include that.

I'm not sure if you had the vaccinations done with the guy in Vancouver, or whether you brought something to him? US civil surgeons here, from the list, have the form you're looking for.
