Vaccination paperwork?

Okay.. here we go...

Im from Edmonton, AB.. and i was a graduate of the Therapist Assistant Program at MacEwan College. In order for us to be accepted into the program, we had to have ALL of our vaccinations up to date. The paper the vaccination nurse gave me was submitted to my school program.

My question is this: What kind of paper work is needed to show that im completed vaccinated (i know i am)? and can i just call my family doctor and book an appointment with him in regards to getting vaccinated for US visa? Will he have the required paper work disposal?

I'm originally from Saskatchewan and I just called Saskatchewan Health and they are able to give me a copy of all the immunizations I have had. Not sure if it works the same with Alberta health but if your doctor was the one who gave you all you up-to-date immunizations, he should have the paperwork.

I just found out that the only vaccinations they have on record are the ones that were done through the schools in Sask, not the ones I had as a baby (thank goodness my mother still has those records) or anything after done by my family doctor.

I was so mad when I went to Capital Health to get mine and they said they had no record of me. Meanwhile another person came in to get theirs (a younger guy) and they printed off his in 30 seconds.

great idea!!! i'm in this same "position" and feel like a walking labo case to be examined on any orifice..even my brains are burning of desire of all those shots...guess it might develop in an addiction...

I believe the health department only keeps vaccination records for 25 years, so if you're older than that they won't have them. If your family doctor doesn't have them then titres are wonderful things.

what's titres? where do you get them?? thanks!!

titres is a blood sample your doctor can take and send to the lab to see what antibodies are already in your blood. That can tell them what you've been vaccinated for already.

I got my vaccination supplement in the mail the other day and im vaccinated for everything (except chicken pox since i've had them already when i was a kid and i know you can just tell them you've had them)

I, however, was unable to find on the supplement that i was vaccinated for Pneumococcal so i made an appointment with the doctor for this wednesday to get the shot.. Is a doctors note saying i got the shot enough? or is there something specific they'll req as proof i got the shot? thanks

Usually anything is OK. Don't they have those official cardstock forms in your area. I know in Ontario we have our yellow cards and I thought that they had something like that in Alberta.

Don't need Pneumococcal, unless you're two years old or younger

Here's a list of the vaccinations you'll need for your appropriate age group. Most often they're MMR, TD and chicken pox.
