Your green card and the SS-5 form. They may also ask to see your passport and/or birth certificate.

DO NOT I repeat DO NOT give up your old SSN card until you get the new card and it is not required to be issued a new card. Say your dog ate it. LOL

If the applicant has a card and requests a replacement card (duplicate or corrected) or a new SSN:

Ask the applicant to return the old SSN card. (If the applicant does not have the card with him or her, remind the applicant to return the old card to SSA for destruction or to keep the old card in a secure place [not in a wallet or purse]).

NOTE: Do not forcefully confiscate the SSN card if the applicant refuses to relinquish it. Similarly, return the old SSN card if the applicant requests that it be given back.

When I went to the SSA office nearest to me to get a new SS card without the DHS condition, they gave me back my old SS card without me having to ask for it.

When I applied to change the name (different case but still), I don't recall being asked for the old card. In either case, I lost it a long time ago, so I could not have given it to them anyways. Just tell them you don't have the card on you or it's been misplaced.

Same for me... when I applied for new one with different name... they did not even asked for the old one... I just filled the number on the original name on the form...

That was it... I currently have both...

They took my old card before they gave me the new one.. but it was not to remove conditions but to change my name...

I am wondering.... will they remove the conditions if my green card is only good for two years ???

For every person that didn't have their card taken or handed over unknowingly, there is an equal number like yourself that left the office without their card.

As far as SSA is concerned a permanant resident is a permanant resident. I-551 stamp and passport or I-551 card will do it. In fact when you have the 2 year I-551 card is the time to get the unrestricted SSN card, since at some time during the removal of conditions process your I-551 card will be expired and you have a restricted SSN card, leaving you without proof of work authorization.

On the application is says if u have a ssn list the no. and the name it was issued in and a different date of birth, what date of birth should i give i didnt get that part.

If you are talking about question 13:

If the date of birth you show in item 6 is different from the date of birth you used on a prior

application for a Social Security card, show the date of birth you used on the prior application and submit evidence of age to support the date of birth in item 6.

Otherwise, leave it blank. That's only for people correcting their date of birth.
