I-693 and I-864 questions

Hi Everyone

I got married to a US citizen two months ago and filed for an adjustment of status and I-130. I got the fingerprints done but today I got a letter from immigration saying that I need to fill a supplemental form of I-693. Does anyone know where this is? I just have I-693 form with me that I need to take to the doctor. What part of that do I need to fill?

Another question is about I-864 my husband filed I-864EZ but did not include yet W2 form so will we still need to submit I-864 besides W2 form??? or IRS transcript??? Do we need any other forms relating to these two forms?

Sorry for so many questions. I would appreciate your help :)
