Hi Grid,

Yes, you need to fill out the entire form honestly. Leaving this line # 9 blank may cause an RFE and longer delays.

Yes, I intend to honestly answer question 9. I am just not sure whether to use my ex-wife's madien name or her married name, which is my name. Thanks for your response.

I would put her maiden name.
