If you want your married name on Green-Card then, use married name on the I-485 form part 1, you put maiden name in part 3 of the form under name on departure record I-94.

Please use the example forms: http://www.visajourney.com/forums/index.ph...p;page=examples

Keep in mind, to change name on a green-card you file I-90 and pay a $370 fee.

File for AOS using the name that you want on the green-card.

Okay, but this raises another question about traveling with the green card. Probably the passport will still have the maiden name, wouldn't it make sense to have the green card in a maiden name so you can travel with your passport?

Book airline tickets using name in passport (Maiden Name) and when entering the USA you show green-card, if any questions come up you can show a copy of marriage cert.

Also US Residents traveling to and form Canada only needs the green-card, so in this case the airline tickets can be booked using married name, the name on the green-card.
