Hello,

I would like to know if someone from Atlanta had their civil surgeon visits?

I have my immunization record when I came to US as a student 6 years ago. I will take that paper to the civil surgeon to show my vaccine history. Can u suggest a civil surgeon that is not going to cost too much too fill out the form?

PS: I have a H1B visa right now and am getting married in January.

In your case you may have to do full medical to adjust status, how long ago was the medical for your H1B visa?

I don't remember having a medical exam for my H1B visa. I was already in the US with OPT after F1.

I guess I need to have a full exam. Any civil surgeons around Atlanta that is not going to cost too much?

Use the CIS Civil Surgeon locater, and call around, most on this board is familure with the vaccination supplement and try to avoid getting burned for the full medical.

I went to the American Civil Surgeon Center, USCIS Civil Surgeons, 233 Mitchell St., Suite 100, Atlanta, 30303; 404-521-3662. It cost me $50 to have the immunization records transferred to the vaccination supplement in March 2006. The form is "Supplement Form to I-693" and is identified as such on the invoice. They also provided me with a copy of the form. They fully understood what I needed and didn't try to tell me otherwise.

I phoned and got a 'time to come in' but it wasn't really an appointment. The medical assistant prepared the form then took it to one of the surgeons doing physicals for review and signing. There were a LOT of people there for their physicals, but I didn't have to wait long. There is on street parking on Mitchell St. The building is actually very near the corner if I remember correctly.

Again the OP came here on a visa other than K-Visa, and needs the full medical and supplement, so fee will be much higher than the $50 you indicated.

I just received a call from my Student Health Services who should have had my records. They say they don't have it anymore! If I somehow cannot find that document I would take all those vaccinations again. I guess I am not lucky :)

Yes, just realized reading the post over that she would require the full medical. Well, that address does full medicals too! Call around and find out who will give you the best price.
