K1 to AOS: vaccination supplement not in sealed envelope

I searched on this forum for vaccination supplement information, and I see many people mentioning vaccination supplement being in a sealed envelope. But all I have is a piece of paper I received at a clinic when I had my medical exam for K-1 application in my coutry, and it is not in a sealed envelope. They just handed me one...

I gave the immigration officer at the port of entry a big sealed envelope that I received from the embassy after my interview, but you guys are not talking about this, right? (I did not get it back, of course).

Could anyone tell me if this is going to be an issue, please?

You need to take the information you got from your medical to a civil surgeon here in the US.... the will copy the info about your vaccinations on to the vaccination supplement seal it in an envelope and give it to you to include with your AOS packet.....

You're right, we're NOT talking about the sealed brown envelope that you submitted to the immigration officer at the port of entry. We're talking about the I-693A vaccination supplement form. To do this, you have to bring that piece of paper you received at a clinic when you had your medical exam for K-1 application in your country to a USCIS certified civil surgeon who will transfer the vaccination information from that piece of paper to the official I-693A form, sign it and place it in a sealed envelope.

Don't worry, the civil surgeon has a copy of the I-693A form. Also, if you're vaccination is incomplete (ex: you have no previous Td shot record), the civil surgeon will administer it so you can comply with the requirements. Try calling before you go to any civil surgeon and inquire about the rates. Not all of them are well-informed about this especially as far as K-1 visa holders are concerned. :blink:

Okay, I think the form you are talking about is the DS-3025. This is given to the beneficiary at their medical in their home country. This is not the same thing as the Appendix A Supplemental Form To The I-693 (called the I-693a here on VJ) that you send in with your AOS.

To get this Appendix A Supplemental Form To The I-693 you will need to go to a civil surgeon in your area, taking the DS-3025 with you. They will then transcribe the information on the DS-3025 onto the supplemental form (which they provide) for you and seal it afterwards. They normally give you a copy for your records too.

Once you get this you can send it off with your AOS application. If you don't send it off with your application you may get an RFE for it or, if interviewed, you should bring it with you to the interview instead.

I did this 3 days ago and I know exactly what I'm talking about on this issue. What you received in your home country clinic is a copy of the vaccination supplement that was related to the K-1 medical examination.

What you have to do is go on www.uscis.gov and find the list of civil surgeons. Find the ones in your area and make an appointment. When you're going to go there bring the I-693A vaccination supplement with you (although they should have it there too. I just brought mine). When you get there let them know that you will need the vaccines transferred from your vaccination supplement (from your home country) to I-693A. Bring any other type of vaccination records with you too. It's easier for the surgeon.

They will give you the I-693A in a sealed envelope and a copy for your records.

If you will send in the one that you have from your home country it could work, but it could cause a delay too with them sending you an RFE.

you will not need a new medical exam when you had yours within 12 month of filing for aos.

I am glad I asked because I thought I just needed to turn in DS-3025 with my AOS...

I will call a civil surgeon in my town and will try the one in Texas, too.

Because my wife had all her necessary vaccinations at her K-1 medical, we just sent in the DS-3025 figuring the worst that could happen is that they would tell us to get the supplement from a civil surgeon. We had no problem and even at our AOS interview the interviewer looked at the DS-3025 and said that it was fine. Glad we didn't pay the outrageous prices the civil surgeons in our area wanted.

I just sent off my wife's AOS paperwork on Friday. I sent a copy of the DS-3025 as well. If you read the I-485 instructions, it actually doesn't refer to the I-693, it just says "vaccination supplement".

To be fair, there have been cases which have received RFEs for submitting only the DS-3025 and not the I-693A, and other cases which shrug and say that the DS-3025 is enough. To be on the safe side I'd get the I-693A so that you don't waste time with the RFE - the doctor in Texas can do it for a very good price, and it only takes a few days through the mail.

I'm with Neonred on this one. I can't recommend people send in one or the other. But when you actually look at the 693a supplement and compare it to the K1 visa page, they're the same page.

I say what's the difference. I paid $500 for two of those darn K1 medical tests. If that doctor was certified for the US government to let us immigrate, they should be just fine as far as passing the AOS test.

If you choose to visit a Civil Surgeon and have the I-693A Supplement completed (as per AOS instructions for K-1), make sure that their particular office is willing to complete the I-693A without a medical exam. Many Civil Surgeons are not aware that K-1's (that have had their medical exam withint a year) are only required to complete the supplement. We went through 30 different civil surgeons before we found one that was willing to simply do the supplement portion.

Texanadian, I'm with you on the Medical Exams, costs and authority to administer the tests....I will say that unfortunately at some consulates, specifically Ciudad Juarez, they do not give you copies of the Medical Exam, nor do they allow you to see them. When you're trying to immigrate, who the heck is going to cause a stink about getting a copy when you are at their mercy- not us. I'm sure at some point if I am ever incredibly curious about the contents of that medical report, I can go through the steps of FOIA.

Two points here. First, this will only work if all required vaccinations are up to date. Second, we tried very hard to get the vaccination supplement like most other people. We had sent in the DS-3025 with the idea that we would see the civil surgeon and have the supplement ready for the interview. We received notice of the AOS interview just about two weeks ahead of time. Called all CS's in our area and I would NOT pay $300 for the paperwork. Did find one CS at the local health clinic that would do it for $25, but they could not see us for over three weeks. (Didn't know about the doctor in Texas at that time) We decided to go to the interview and see what would happen thinking we might get an RFE and have to make an appointment at the clinic and send in the paperwork three weeks later. By that time a few weeks wouldn't matter at all and it was better than spending the $300 if it wasn't necessary.

At the interview the officer looked over everything and I saw him look at our DS-3025 and comment that our medical was fine and all vaccinations were OK. What a relief. Worked for us.

For $10 I would, however, say send to the doc in Texas and have it ready just in case, but don't put off sending your package in because you haven't done the supplement.

Just as a bit of background to try to shed some light on this issue..... Some countries allow K non-immigrant visa holders to defer the vaccinations until after they arrive in the US, which is why they request the I-693A with the AOS submission. If the beneficiary's vaccinations were completed as part of the visa process, they are not required to submit the supplemental vaccination form because all that information is enclosed in the brown envelope. This is not to say that someone processing the I-485 will not request the information in an RFE. As we all know, not everyone employed by USCIS has the same understanding of the rules and regulations applicable to K-1 visa holders.

In my own case, I submitted a copy of the DS-3025 and was approved without a problem.
